0068718152 2014-04-02 Drill / Deepen / Reopen No Violations Status check inspection of the work over process found the current drilling depth to be 6,164 using a 6.250 direction drilling assembly, the company is unable to get their required build rate of 30 degrees and has requested permission to plug the well back to the vertical section and re-drill the borehole using a 8.5 diameter drill, permission was granted to make the changes needed.

On 04/05/14 a total of 4 plugs were set through 100 of 2.875 tubing under the drill pipe from 6154 to 2477 in 4- 900 increments with the plug being tagged at 2540, the well bore drilling was the resumed using a 8.5 directional drilling assembly with a current drilling depth of 4232.

1395932094 2014-03-27 Drill / Deepen / Reopen No Violations Status check inspection on the drilling process found the 9.625 casing has been set to a depth of 2750 and the company has drilled out from under the casing to begin drilling the 6.125 production well bore, a formation integrity test was conducted using 8.4# mud by increasing the annular pressure to 1050 psi giving a formation integrity equal to 15.8# mud.
1397524793 2014-04-14 Drill / Deepen / Reopen No Violations Status check inspection on the drilling progress found the 8.5 production borehole has been drilled to a depth of 13,255 at a azimuth of 135 degrees, the well has had 20# casing ran with a float shoe and collar along with a rapid start valve used and proper centralizer placement demonstrated, Halliburton cement services has finished cementing the production casing in place with a calculated top of cement at 1,467.
1405373520 2014-07-12 Completion Testing No Violations On site along with Deputy Chief Bob Worstall to witness Halliburton pump kill fluid into well at 3000 psi and pressure started to drop and pump rate was increased and after pumping 135 bbl of fluid well was static and on slight vacuum and decision was made to rig up onto the next well.
1405426705 2014-07-14 Completion Testing No Violations Status check follow up to the recent location fire. On this day the said well had the well completion control head and piping removed and Horizontal wireline service set a 10,000 psi rated retrievable bridge plug at a depth of 5,000. Inspection of the North East corner of the well pad working area observed some chemicals released during the fire were still migrating to the corner of the location and notification was made to Stat oils company men on site and a 3rd party contractor was utilized to recover the product with a morning report on 07/15/14 from the vacuum truck operator indicating he recovered approximately 40 barrels of product. My observation and inspection of the plastic underlying containment has found the plastic containment primarily still intact in the North East corner and retaining fluid allowing for the recovery of the released product.
1405541324 2014-07-16 Completion Testing No Violations Status check on site inspection to witness the changing out of the well tubing head and master control valve, a backpressure valve was set in the well head assembly on 07/15/2014 giving 3 levels of well bore protection to allow the well head assemblies to be changed out, the assemblies were pressure tested to 5,000 psi. Wells number 5-6&7 were in process of being stimulated on 06/28/2014 when a equipment fire broke out on location causing the chain of events leading the wellheads to be changed for structural integrity reasons.
1515625401 2018-01-10 Work Over Reconditioning No Violations On site status check inspection found the company preparing to secure the well heads and subsurface well bores in anticipation of bringing in a drilling rig to drill additional wells on the pad. No tubing is present in the well bore and the company has set a 8,000 psi pressure rated retrievable bridge plug in the well bore at a measured depth of 5,000 and set a back pressure valve in the casing mandrel section of the well head. A R-24 night cap has been installed on the well head. All production equipment has been removed from the well head.
2098287310 2013-12-26 Casing String Witnessed No Violations On 12/26/13 Orion drilling is now rigged up on the hole and 483 of 13.375 surface casing was ran and cemented to surface by Halliburton cement service with 15 barrels of cement returns to surface and a sting in cement collar ran.
