0099065213 2019-01-28 Production Wells No Violations On this day, I found the pad gated with signage at the entrance. There are (7) producing wells, gas processing units, (6) 400-barrel steel production tanks within steel containment, (4) 30,000 gallon horizontal condensate pressure vessels in containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Concrete barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 0 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 0 psi.
0630614493 2013-08-02 Casing String Witnessed No Violations Inspection to witness the running and setting of the 13.375 surface casing. 655 of 54.5# J-55 casing was ran and cemented back to surface with 20 bbl of cement returns to surface. The mix water was tested and showed no high chlorides. A anti collision borehole deviation survey showed the well bore is deviated less than 1/4 of a degree.

1092487639 2013-09-09 Casing String Witnessed No Violations Status check on the running and cementing of the 5.5 production casing, 11,900 of 20#casing was ran with solid centralizers on every collar up to the top of the horizontal curve and bow string centralizers on every collar from the top of the curve up, A float shoe and collar was ran along with a rapid start valve. 60 barrels of clean spacer was ran prior to cementing and 2520 sacks of class A- 15.0 lb -1.3 Yield cement was ran.

1371573692 2013-06-18 Drill / Deepen / Reopen No Violations On 06/17/2013 the said well was spudded and 100 of 20 conductor was set and cemented to surface. On this day I reviewed all the 7 wells that have been spudded and have had conductor set and found that the top hole company had mixed up the #7 & #5 wells surface identification and cement job paperwork, The company man checked the engineered plans and corrected the mistake. It will also be noted that they are setting 75 of 16 mouse holes inside of each cellar, these are to have steel plates attached to the bottoms of the pipe and the exterior boreholes are to be grouted through a 1 string to surface.

1699091908 2015-01-14 Preliminary Restoration No Violations Shale well preliminary restoration status check inspection found the said well has had the drilling and completion process completed and multiple wells are producing on the well pad with the following production related equipment in place on the location. There are 7 total wells on location each with individual gas processing units and a total of 4 -3,000 gallon 150 psi cylindrical vessels used for condensate along with 4-400 barrel production water tanks. Preliminary Restoration passed.
