0115584436 2016-02-16 Preliminary Restoration No Violations Status check inspection found the well head equipped with (2) 5.185 master control primary gate valves assembled on the B-section well head along with (2) tubing head master valves and a primary control hydraulic emergency shut down in line vale on the tubing head stack located between the 2 tubing manual valves. There is a sand trap separator hooked into the production outlet flow line near the well head supported by a 100 barrel production tank located on location near the well head. The well does have production tubing in it and the well is currently in production with a total of 10- 400 barrel production tanks located on a production pad adjacent to the well pad with 7 gas processing units along with a gas compressor also located on the adjacent production pad. The well was turned over to the companys production department on and after 12/17/2015. Addressing the preliminary restoration- upon inspection of the well pad it was observed that the primary perimeter boundary affected by the well pad construction is secure and has no stability or erosion issues with vegetation established on the well ad perimeter. The well pad itself has earthen material that was stockpiled after a well control incident during December of 2014, According to Sam Miracle with Triads Engineering Department the material is slated to be addressed once the weather breaks this spring. There is also a large portable polymer lined water tank on the well pad location that is being emptied of all fluids currently and is to be disassembled and removed from the locating in the future.
1042607624 2019-03-12 Production Wells No Violations Status check inspection found the well equipped with a lower A and upper B section with 2 diameter riser pipes attached to the service ports on the A&B sections. The riser pipes are equipped with valves and gauges for the purpose of annular pressure monitoring. The A section pressure gauge reads 150 pounds and the B section pressure reads 250 pounds. The 5.5 production casing well head section flow cross is equipped with a gauge that reads 1,700 pounds pressure. The well head is equipped with a production tree and lubricator head with 1,400 pounds pressure on the production tree. The well is flowing through a sand trap separator. The production facility is situated on a separate production pad and consists of (10) 500 barrel capacity tanks along with 5 gas processing units. On site on this day were company representatives Neil Ingram, George Winland and Ryan Lori.

1201390161 2017-12-07 Preliminary Restoration No Violations Preliminary restoration status check inspection found the area affected by the well pad construction perimeter boundary to be stable with vegetation established. There is a 2nd pad site that us used as a production pad that the lease road to the pad runs through that also has the perimeter established with vegetation, on the second pad is the tank farm that consists of a total of 10 steel design 500 barrel fluid capacity steel tanks situated inside a polymer lined steel containment area. There is also a large gas compressor and 5 gas processing units on the second pad site. Addressing the primary pad site there are 5 wells completed on the pad with all 5 wells found to be in operation on the pad. Proper well identification is posted on each well and at the tank farm battery site. The lease road is of stone construction. Bruce Carpenter with Eclipse Resources recently contacted me and indicated that Eclipse has taken over ownership of the wells and equipment.
1313773414 2015-05-26 Completion Testing No Violations Status check inspection on the well flow back completion process found Fesco Well Control Company on site conducting well flow back post stimulation operations. Conversation with the Flow Back Engineer with Fesco indicated to the fact that today would be the final day for flow back and the company would be released and breaking down their equipment in order for Triad to begin normal production operations. On site with me today was ODNR Engineer Tech Andy Thomas who agreed the Preliminary Restoration could not be considered completed until the company removed the 2 large piles of soil placed on location during the well control event during the month of 12/2014.
1403595391 2014-06-24 Casing String Witnessed No Violations On 06/24/2014 Warren Drilling rig 12 is now on the hole and has drilled a 24 diameter surface borehole to a depth of 530 and ran 504 of 20 casing with Halliburton cement services cementing the casing to surface using 620 sacks of Halcem cement with 40 % calculated open hole excess and 37 barrels of cement returns to surface.

1836896581 2014-04-23 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that Express Energy Services had drilled a 32 inch diameter hole to the depth of 100 feet and ran 100 feet of 26 inch conductor pipe. Express Energy Services cemented conductor pipe with 250 sacks of Class A cement with 2% CaCl and .5 #/sack of floseal. Had good returns of 5bbls and cement stayed to surface.
