0138349072 2015-06-18 Preliminary Restoration No Violations Preliminary Restoration follow up inspection found the restoration status is still showing up as active in the database and was signed off by myself on 02/26/2015. I once again have added the well GPS coordinates into the system also. Inspection of the well head found the following equipment present 5.5 production casing with the production valves open and selling into a 2 sales line. A total of 4 -400 barrel capacity tanks are present in a plastic lined metal constructed dike area. Chloride levels were tested at 4 random locations across the well pad with no levels showing up over 121 parts per million on a low level Quantab field strip test. Preliminary Restoration once again passed on this day.
0204425221 2013-12-17 Casing String Witnessed No Violations Status check inspection found the well bore total drilling depth of 15,335 has been reached and the 5.5 production casing has been set at 15,316. Baker Hughes cement services are on location beginning the long string cement job.
0218227729 2013-12-05 Drill / Deepen / Reopen No Violations The Drilling rig Falcon Rig # 17 has skidded over on to the well, The 9.625 casing was previously set and only the production borehole is left to drill. Some issues I addressed on my last inspection with containment have been addressed.
0218956085 2013-12-03 Drill / Deepen / Reopen No Violations Status check inspection found the drilling rig Falcon Rig 20 rigging back up on the hole preparing to drill the horizontal well bore section, Orders were left with the company man on sit to straighten up the containment that has been damaged and clean up the fluids mixing area.
0611670921 2018-03-13 Production Wells No Violations On this day, I found the well equipped with annular pressure monitoring devices. The surface pressure gauge is reading 300 pounds per square inch, and the 9.625 gauge is reading 1,600 pounds per square inch. There are (4) wells in production on this pad. All wells have concrete barriers surrounding them. The facility is gated with proper signage. There are (4) 400 barrel steel tanks within containment at the front right-side, prior to the pad, with an electronic shutdown button. There is a midstream facility at the front right-side of the pad, and (4) gas processing units beside it. The wellheads are not properly identified, but it will be noted that I spoke with Antero representative, Jonah Turner, during my inspection on another pad, and informed him to make sure that his operators have their wellheads identified with proper signage.
0719449694 2013-09-23 Drill / Deepen / Reopen No Violations Inspection and status check on the drilling process found the current drilling depth to be 7091 using a 12.250 bit with a projected total depth for this segment to be 7750 setting 9.625 casing.
0831922803 2013-09-16 Drill / Deepen / Reopen No Violations Inspection found the drilling company not able to properly deviate out of their recently set kick plug and notification was made to me that they shall clean up the hole and reset the kick plug through tubing attached to their drill string.
0886941788 2013-09-10 Plug / Plug Back No Violations A 17.5 hammer bit was lost at 2070 and a plug was set at 2007 up to 1650 using 600 sacks of class A cement. The 13.375 casing is to be set on top of the plug.
0891306965 2013-09-12 Casing String Witnessed No Violations On site inspection to witness the cementing of 1543 of 13.375 casing, this 1st intermediate string was set short due to a lost 17.5 bit at 2070. The well bore was plugged back and the casing set under the understanding it was cemented to surface and the 2nd intermediate shall be cemented to surface. Baker and Hughes used 1357 sacks of type 1 cement and saw 75 barrels of good cement returns to surface. The casing shall be pressure tested during the BOP test. The Division Deputy Chief Dave Claus and My direct Supervisor Rocky King along with the permitting section lead man Steve Opritza was aware of the casing program change.
1375446743 2013-08-02 Casing String Witnessed No Violations The said well was spudded on 07/31/13 with 100 of 24 conductor being set. On 08/02/13 I witnessed the cementing of 360 of 20 surface pipe in a 23 borehole with 3 centralizers ran and 10 barrels of good cement returns to surface. The mix water was tested and showed no chloride levels. Baker Hughes cement services were the cement contractors and Zigenfuss were the drilling contractors.
1378736707 2013-09-09 Drill / Deepen / Reopen No Violations Inspection and status check of the drilling progress found the current drilling depth to be 2070 using a 17. hammer bit. The company man indicated they are having trouble making hole and have pulled the drill string out of the hole for a bit change.
1378771050 2013-09-09 Drill / Deepen / Reopen No Violations

-1st intermediate short of their permitted length, cementing it to surface , They will then deviate around the lost tool while drilling the 2nd intermediate. I spoke with Dave Claus and Rocky King and we are in agreement that as long as both intermediate casing strings are cemented to surface this would be allowable. This all took place after 5pm and due to the aggressive nature of the red rock collapsing we had to give them some sort of direction to approach this matter while the hole was still workable.

1380183379 2013-09-25 Casing String Witnessed No Violations On site inspection to witness the cementing of 7810 of 9.625 intermediate casing. Baker Hughes cement services cemented the 2 stage cement job with 60 barrels of cement returns to surface and both stages pressure tested.
138349072 2015-6-18 0:00: PL No Violations 1.00
1386776779 2013-12-11 Drill / Deepen / Reopen No Violations Status check inspection on the current drilling progress found the current drilling depth to be 13,141 using a 8.5 bit assembly. The last directional survey was at 13,019 with a Azimuth of 149.92 and Inclination of 88.58 degrees and a True vertical depth of 8,921, In review of the last submitted mud report the mud weight was 11.40# and the viscosity was 94.0, I discussed with the company man the mud weight being low and viscosity being high and asked Him to review this and make corrections accordingly.
1399315256 2014-05-05 Completion Testing No Violations Status check inspection found Gofrac services rigged up on the hole rigging up to begin stimulation within the next 24 hours, the hole is being perforated between 15,239 and 15,109 with 6-10 shot clusters.
1599337506 2013-09-18 Drill / Deepen / Reopen No Violations Status check inspection on the drilling process found the current drilling depth of 1583 and the drilling company running back in the hole with 12.250 directional tools after a drive motor failure.
1735257338 2018-10-04 Production Wells No Violations Status check inspection found (4) wells present on the pad with all wells found to be in production. The lease road surface is stone construction and in good repair. There is a gate across the lease road with owner and well pad information present. The production facility is also along the lease road and not on the well pad. The production facility consists of (4) steel construction tanks capable of holding 400 fluid barrels capacity each. The tanks are situated inside a polymer lined steel containment dike area with hazard warning labels present. There is also an earthen berm around the production facility. Situated on the well pad is a large gas compressor. Addressing the well head the well is equipped with a lower A and upper B section with a production tree present. The production valves are in the open position. 2 diameter riser pipes are present with valves and gauges attached for annular pressure monitoring. The 13.375 well head section showed 80 pounds pressure and the 9.625 well head section showed zero pounds pressure. Also noted the South East slope of the well pad does have an earthen slip that is being addressed by the Division Engineering section. The slip has required 10,000 to 12,000 cubic yards of material to be removed. The company is remediating the slip under emergency stabilization guidelines under the well construction rules.
1970798295 2015-02-26 Preliminary Restoration No Violations Preliminary Restoration status check inspection found 4 wells total present on this pad at this time with the well head equipped with a production tree with the production valves in the open position selling into a gas processing unit equipped with a mechanical sand trap and USA Compression gas compressor set on the well location with a inline fluid separator and dryer system present. 4- 400 barrel production tanks are present near the well pad entrance supporting the wells on the pad, all located inside a mechanical dike area with proper well identification present at both the well head and tank battery area. Preliminary Restoration passed on this day.
