0191627329 2014-02-20 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that Nomac Drilling Rig #359 had skidded from the 34111244230000 Shroyer Unit 4H and was preparing to trip in the hole with an 12.25 inch diameter bit and drill out the 13.375 inch float shoe at 2392 feet continuing down to approximately 8900 feet and running 9.625 inch 2nd intermediate string of casing.
0511425115 2015-02-10 Preliminary Restoration No Violations Status check inspection on the Preliminary Restoration found the well in production with 2.875 tubing set in the well bore and proper well identification posted on the well head. There is 3 -400 barrel capacity production fluid storage tanks on location along with a 100 barrel tank, all located in a metal dike area with a plastic type containment dike. There is 2 gas processing units along with 2 mechanical separators located on the far East side of the well pad to support both wells on the pad now producing. Proper well identification is posted at both the well head and tank battery. The Preliminary Restoration was passed on this day. No violations noted.
0677427383 2013-11-06 Drill / Deepen / Reopen No Violations Status check inspection found the drilling company Alpha Hunter Drilling rig # 9 has drilled into the top of the coal mine void at 757while drilling their mine string. They have pulled out of the hole with the drill string and are going back in with recovery tools to try and recover iron that is in the hole left from the underground mining operation.
0757108480 2014-03-11 Drill / Deepen / Reopen No Violations On the day of my inspection accompanied by ODNR Inspector Randy Akins, I witnessed that Nomac Drilling Rig #359 was drilling their lateral with an 8.5 inch diameter at 16155 feet with a projected td of 19047 feet. Site Inspection revealed no violations.

0928235562 2014-06-24 Completion Testing No Violations On the day of my inspection I witnessed that Producers Service Company was on site and stage sixteen of twenty nine total stages was in progress. Average treating pressure at the time of inspection was 9373psi, 40/70 mesh sand @1.5# and average rate 65bpm. Completed walk around with Unconventional Completions Foreman Alex Pottmeyer revealing no violations.
1312341837 2018-08-28 Preliminary Restoration No Violations Preliminary Restoration found the area affected by the well pad construction meets ORC 1509.072 with vegetation established and erosion control in place. The well pad and lease road are of stone surface construction and in good repair. 4 total well are now present on the well pad. The said well is equipped with a lower A and upper B section well head assembly with 2 diameter riser pipes attached to each well head section, the riser pipes are equipped with valves and gauges to monitor annular pressure. The A section gauge which monitors the pressure between the 13.375 and 9.625 diameter casing string reads zero pounds pressure. The B section gauge which reads the pressure between the 5.5 and 9.625 diameter casing string reads 600 pounds pressure. The 5.5 internal casing pressure is 800 pounds and the flow line pressure downstream of the well head choke is 353 pounds. The tubing head pressure is 392 pounds pressure. On the left rear side of the well pad is the production fluid facility consisting of (3) steel construction 400 barrel capacity tanks along with a single 100 barrel tank, all situated inside a polymer lined steel containment dike area. There is a 3rd party midstream pipeline point and compressor on a separate pad at the rear of the location.
1377124667 2016-01-19 Production Wells No Violations Status check inspection to document the well head working pressures. On this day it was observed that the production tubing head had a pressure of 1,098 psi with the 5.5 showing 1,387 psi at surface internally- the annular of the 5.5 showed 1387psi and the annular of the 9.625 showed 13 psi. The production tubing sales line showed 1,068 psi. The annular 5.5 casing monitoring poind does have a 1 line ran into the production sales line to releive annular pressure.
1383403310 2013-11-02 Casing String Witnessed No Violations On 11/02/13- 420 of 24 surface was set and cemented to surface with 100% cement excess ran and cement returns 85 barrels before total displacement. Alpha Hunter rig # 9 will drill and set the surface, mine string and 1st intermediate.

1394722496 2014-03-13 Drill / Deepen / Reopen No Violations Status check inspection of the drilling process found the current drilling depth to be 18,730 using a 8.5 diameter drill bit with a projected total depth of 19,047 and true vertical depth of 10,799 and azimuth of 154.77 degrees and inclination 88.98 degrees.
1398890089 2014-04-30 Drill / Deepen / Reopen No Violations Notification was received today from Kyle Bradford with Eclipse Resources regarding the recent bond log ran on 04/26/2014 on the 5.5 production casing to verify the cement top on the annular side of the casing, the bond log showed an estimated cement top in the horizontal section at 11,000 and the tail of the 13 pound per gallon 1.58 yield at 15,800 with a total of 18,717 of 5.5 production casing ran leaving 2,917 of total coverage in the horizon section, notification was made to my supervisors and a meeting will be set up once all documentation is in place.
1402524209 2014-06-11 Completion Testing No Violations On this day, I observed several companies on site rigging up to begin the stimulation process on the 2 known wells on location. I contacted Kyle Bradford with Eclipse and he indicated they planned on stimulating the well. I informed Kyle that on 04/30/14 I had made notice to him that the production casing cement job had inadaquate coverage and befort they (Eclipse) proceeded on with the completion process we would need to review the Bond Logs and completion records to ensure they had proper zonal cement coverage. At that point I contacted my supervisor Rocky king who notified Kyle Bradford that they were not to proceed with the stimulation process until the Division reviewed and approved their completion records and formation cement coverage as required in ORC 1501:9-1-08 Well Construction rules.
1403128165 2014-06-18 Completion Testing No Violations On the day of my inspection I witnessed that Producers Service Company was on site and fracing had commenced on the said well with stage two out of twenty nine in progress. I met with Company Man Billy Wells and discussed frac and stages. Average treating pressure at the time of inspection was 10240psi, 1/4 # sand and average rate 57bpm. Due to time restraints was unable to witness complete stage. Site inspection revealed that all equipment was in containment.
1816733971 2013-11-18 Drill / Deepen / Reopen No Violations Alpha Hunter drilling rig 9 has completed drilling and setting the 13.375 1st intermediate casing string to a depth of 2420 with 2392 of casing being set and cemented. Nomac Drilling rig 359 shall take over and complete the well bore drilling process.
