0323733555 2018-03-08 Production Wells No Violations On site inspection to document the well head annular pressures. The 13.375 well head has 20 pounds pressure and the 9.625 well head has 2,000 (PSI) pounds per square inch pressure. The production tubing valves were open along with the (ESD) Emergency Shut Down valve. The location has several emergency shut down control points identified at the well pad entrance- at each of the (3) wells total on the well pad - at the production fluid facility and the gas processing units. The well is producing of the tubing head assembly. On this day I spoke with a company HSE representative identified as Bruce Carpenter and we discussed the annular pressure on this well and the rest of the wells on the pad. I have asked the company to monitor and record all annular pressures and address any problematic wells that may have pressures present that could compromise the well head or casing integrity.
0981850491 2015-02-24 Casing String Witnessed No Violations Status check inspection on the current drilling process found H&P Drilling rig 531 on location and the 12.250 1st intermediate well bore total drilling depth recently reached at 8,916 and 8,863 of 9.625 40# casing set with a 2 stage DV tool set at 2,515 with proper centralizer placement demonstrated, Schlumberger cement service is on location to cement the casing with a lead and tail class A cement mix to be ran below the DV tool with circulation established prior to cementing by the means of the drilling rig mud pumps and 35 barrel of gel spacer ran ahead of the bottom section cement job.
1112380789 2015-03-02 Drill / Deepen / Reopen No Violations Status check inspection on the current drilling process found H&P Drilling rig 531 on location with the production wellbore 8.750 total drilling depth reached on this day at 17,276 in the directional azimuth of 151.28 degrees with a true vertical depth of 10,150. The company will continue to condition the borehole by circulation method for 8 hours pulling one drill stand every hour before pulling the drill string out of the hole and running the 5.5 production casing. Primary containment is on place on location and all location perimeter drains are in the closed position during drilling operations per company policy.
1419961741 2014-12-30 Casing String Witnessed No Violations The said well was spudded on 12/29/2014 by Precision Rat hole Services with a 120 x 30 conductor borehole being drilled and 120 x 24 conductor pipe ran and set. Universal well service is on location to cement the conductor using 305 sacks of type 1 2% calcium .25 pps flake 40% open hole excess cement. The mix water was tested prior to cementing with the chlorides levels reading 56 parts per million and PH level reading 7.
1424019481 2015-02-15 Casing String Witnessed No Violations On site inspection to witness the cementing of the 18.625 surface casing cement job. H&P Drilling rig 531 is now on location and has drilled a 21.750 surface borehole to a depth of 357. Schlumberger Cement services is on location to cement the casing using 370 sacks of Class A cement with 20 barrels of gel spacer ran ahead of the cement and circulation established to surface prior to cementing with good cement returns to surface and the casing pressure tested upon the landing of the cement plug.
1424222739 2015-02-17 Casing String Witnessed No Violations On site inspection to witness the surface casing cement job found H&P rig 531 has drilled a 17.5 x 1,670 surface borehole and ran 1,657 od 61# J-55 casing with a guide hole and float collar ran along with proper centralizer placement being demonstrated. Schlumberger cement service is on location to cement the casing using 1240 sacks of class A cement with circulation established prior to cementing and good cement returns to surface, the cement water was tested for PH & Chlorides.
1438715828 2015-08-04 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the affected area depicted in the pre-site has been properly restored with proper vegetation and erosion control present, the well site meets ORC 1509.072. Addressing the well head -The well drilling and completion phase is now complete and the well is producing with 2.875 production tubing set in the well bore. There is 3 gas processing units on location one for each existing well along with gas metering equipment. There is a metal type poly lined containment dike supporting the 3-400 barrel and one single 100 barrel production tanks on location. Proper well identification is posted and the location is secured by a 6 woven wire fence with a emergency shut down button located at the entry point to the well pad.
