Inspection of the lined sumps constructed outside the well pad at global positioning system (GPS) coordinates 39.776921 -80.925483 revealed no fluid and at 39.776714 -80.925548 revealed a pH of 6 and chloride levels of 643 parts per million (ppm) and a pH of 6 and chloride levels of 997 parts per million (ppm). Field tests performed on the well pad near the working tanks at the GPS coordinates 39.775895 -80.925601 revealed a pH of 6 and chloride levels of 3296 ppm. ESM was then instructed by Mr. King to dig a collection sump near the working tanks to collect fluids to be removed until frac is completed in 10 days. Mr. Lois performed initial conductivity tests within the well site boundaries to determine the impacted areas from the release and to determine a plan for further testing.

Fluid in pit #2 at GPS coordinates 39.776714 -80.925548 was field tested and revealed a pH of 7 and chloride levels of 203 ppm. The fluid found in the northwest sump was field tested and revealed a pH of 7 and chloride levels of 997 ppm. Various puddles were field tested on the well pad revealing a pH of 7 and chloride levels of < 250 ppm. We met with Eclipse EHS Technician Ken Durst and completed walk around revealing containment was being removed. I instructed Mr. Durst to fill the mouse holes in accordance with OAC 1501:9-11-11 and to continue to pull fluids out of the sump until the levels are acceptable.

Proper well identification is posted. Annular pressure monitoring is in place on the 13.375 and 9.625 well head sections with zero pressure on the 9.625 and 200 psi on the 13.375 well head sections. Production related equipment on site consists of 5 gas processing in line heaters and 5 sand trap separators along with a single glycol heater. The production fluid facility consists of 4 steel construction tanks of 400 barrel capacity situated inside a polymer lined steel containment area.

0808539242 2017-12-13 Work Over Reconditioning No Violations Status check inspection found a snubbing unit work over rig set up on the well owned by Deep Well Service with a Quail Service Tools workover blow out preventer stack. The rig is fishing part of the 2.875 diameter production that was lost in the well bore curve between the depths of 10,361 and 11,071 . The company man on site indicated the well bore communicated with another well in the area being stimulated and there is sand in around the tubing string and they had to back the production string off down hole. They have made a clean out run down to the top of the lost string and stabilized the well bore with brine water. They are now pulling the workover string so they may put a fishing neck recovery tool onto the workover string to attempt the recovery process. Once they grab ahold of the lost string they will make an internal clean out run with a coil tubing unit so they can get downhole inside the tubing with wireline tools to free point the tubing. The service rig is situated on polymer containment.

1114919161 2015-08-19 Casing String Witnessed No Violations The said well was spudded on 08/17/2015 by Precision Rat Hole with a 30 x 120 conductor borehole being drilled and 120 of 24 .375 wall 94.62# pound per foot weld connection casing being ran. Universal Well Service is the cement contractor on location to cement the casing using 320 sacks of class A-3% calcium .25 pound per sack loss circulation material with the pipe capacity being displaced with 53 barrels of fresh water and 10 barrels of 6% gel prior to cementing with good cement witnessed to surface. Inspector Brian Sefcik accompanied me on this day.

1477938525 2016-10-31 Completion Testing Violations Noted On the day of my inspection following up on a reported release on the pad from 10/30/16 at 0730, I met with EHS Supervisor Bruce Carpenter and EHS Technician Tyler Bryant. Mr. Carpenter stated that a Crescent employee was filling a 500 barrel produced water tank with fresh water and overflowed the tank by 200 barrels. The secondary containment failed resulting in the water flowing across the pad to the northwest side and an unknown amount of water breaching the well pad. Field tests performed on puddles found outside the well pad at global positioning system (GPS) coordinates 39.776921 -80.925483 and 39.776714 -80.925548 revealed a pH of 6 and chloride levels of 643 parts per million (ppm) and a pH of 6 and chloride levels of 843 ppm respectively. Field tests performed on the well pad near the working tanks at the GPS coordinates 39.775895 -80.925601 revealed a pH of 6 and chloride levels of 4933 ppm. Inspection of the north and west sump valves revealed that they appeared to be shut but the outlets could not be found. Owner/Operator tried to locate the outlets by unsuccessfully excavating. At 12:20 ODNR Engineering Tech Andy Thomas and ODNR Emergency Response Tech Nathan Lane arrived and completed walk around. Owner/Operator has removed 3 roll off boxes of contaminated materials to be disposed of in a proper landfill. Owner/Operator shall supply ODNR copies of the manifests for the disposal and continue to excavate the north and west sump drains to verify the valves are closed. Owner/Operator excavated two collection sumps which will be lined to collect water seeping from the pad. Owner/Operator shall continue to test, collect and dispose of contaminated waters and soils from the pad to ensure that all oilfield contaminates are removed and chloride levels are less than 250 ppm.
1477949314 2016-10-30 Completion Testing No Violations On this day I was contacted by Bruce Carpenter with Eclipse at 13:57 pm and informed they had ran a 500 barrel tank over while pumping fresh water into the tank with a reported 5 barrels of product spilled inside containment and a undetermined amount seeping through holes in the containment. Upon further questioning I was informed some well related product was inside the tank. I informed Mr. Carpenter to notify the divisions one call number and contacted my Supervisor. The producer currently has contractors on site stimulating the well.

Various puddles were field tested on the well pad revealing a pH of 7 and chloride levels of < 250ppm. I completed walk around with Eclipse Consultants Will Ochampaugh and Cristo Giannareas revealing that secondary containment was in place and in good shape and production equipment was being installed at the south end of the well pad.

During my inspection I witnessed Deep Well Services was onsite drilling out the 3411124592 Holliday A 1H. Field test in pit number one at the global positioning system coordinates 39.776921 -80.925483 found chloride levels of 46 parts per million. Field test in pit number two at the global positioning system coordinates 39.776714 -80.925548 found chloride levels of 275 parts per million.

2007509170 2017-03-21 Preliminary Restoration No Violations Preliminary Restoration found the area affected by the well pad construction meets ORC 1509.072 with vegetation established and erosion control in place. The lease road is in good repair and stoned. The location entry point is posted with the owners contact information. The location is fenced with chain link fence. There is a total of 6 wells on the pad 5 of which are complete and all 6 have identification posted. The said well has a casing and tubing well head in place and is equipped with tubing head valves and B section cross valves. There is a Hydraulic operated electrically controlled emergency valve on the tubing head output flow line that has shut down points at the well head and production facility along with at the well pad entry point. Also on location downstream from the well head is a sand trap and line heater and then a mechanical separator. The mechanical separator sends the processed fluid to the production facility that consists of 4 steel construction 400 barrel capacity production tanks located inside a polymer lined steel containment area. There is also 2 dehydration units on location that service the 6 total line heaters on the pad.
