0070856007 2017-02-08 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the area affected by the well pad construction meets ORC 1509.072 with erosion control in place and vegetation established. No pits were erected during the drilling process. The well has been drilled and not stimulated and there is no production tanks in place. The well pad is fenced and gated with a lock on the gate.
0620901631 2017-06-20 Completion Testing No Violations Status check inspection on the well completion process found the 5.5 production casing was perforated and a cement squeeze job administered to the annular side of the well bore. A casing patch was set over the perforations. Halliburton Pressure Pumping and Allied Wireline will move in and complete the well stimulation in the next 10 days.
0697774141 2017-09-25 Production Wells No Violations On the day of my inspection, I accompanied ODNR Inspector William Stolarik. Please reference his comments for a detailed report. Upon inspection we found Fesco on location, (2) gas busters, and (6) 500 barrel horizontal steel tanks within containment. Bill was contacted on September 23, 2017 about a slight leak from containment surrounding the 500 barrel tanks.
0819256043 2019-04-04 Production Wells No Violations On this day, I am accompanied by ODNR Inspector Rick Brown. I found the pad gated with signage at the entrance. There are (2) producing wells, (2) gas processing units, (2) 400-barrel steel production tanks within steel containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Concrete barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 20 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 0 psi. The production records are current.
1056545683 2017-09-25 Production Wells No Violations Status check inspection as a follow up to a reported fluid spill on 09/23/2017 by Company man Darrin Galisky who indicated approximately 3 barrels of production fluid had escaped off polymer containment through a hole in the containment. It was observed on this day that a total of 4 steel tanks of 500 barrel capacity were situated on the well pad providing support to the Fesco Company flow back services who were flowing back the (2) wells on the pad during their initial stages of production. One of the flow back tanks had not had the side dome style access port closed before being placed into surface and was leaking and also placed in a way as to limit access to close the tank port hole. The company has recovered any standing product from the well pad.
1198142473 2016-11-30 Completion Testing No Violations On site inspection to check on the 5.5 production string cement job evaluation. Well Service Group is rigging up a tubing snubbing work over unit and preparing to run in the hole with a 2.875 workover string. The workover well head assembly consists of a manual and hydraulic master valves and a Blow out prevention stack consisting of 23sets of pipe rams and one set of blind shear rams. The company will run in the hole and drill out any cement inside the 5.5 production casing down to the float shoe and then pull the workover string and run a casing bond log to determine if any of the 5.5 annular side is open.
1272008449 2016-08-25 Drill / Deepen / Reopen No Violations The inspection today revealed that the well is drilling ahead. The company is currently drilling with a 17.5 inch diameter drill bit and is at the measured depth of 2227 feet. The company has encountered a volume of oil and gas around 1960 feet and has stopped drilling to fill the bore hole with fluid. Once the company has the amount of oil and gas level controlled they will resume drilling to the first intermediate casing point at 2500 feet.
1471864921 2016-08-22 Casing String Witnessed No Violations On site inspection to witness the surface casing cement job found Orion Drilling Polaris rig 14 on location with a 24 surface borehole drilled to 428 and 372 of 20 94 lb/ft J-55 buttress thread casing ran. Halliburton cement service conducted the inner string cement job by pumping 320 sacks of class A -
1472147789 2016-08-25 Drill / Deepen / Reopen No Violations Status check inspection on the well drilling progress found Orion Drilling Polaris rig 14 on location currently drilling the 17.5 production borehole at a measured depth of 2,027.The company has encountered a large volume of gas and oil at 1,960 and has stopped drilling to load the borehole with fluid and will complete the 17.5 well bore on fluid. Statoil Drilling Superintendent Cameron LaVigne was on location and we discussed their proposed amended casing design; Mr. LaVigne indicated they would be contacting the Divisions permitting section with amended design proposals for any wells drilled moving foreword. Mr. LaVigne was also informed that the company was required to notify the Division and the local emergency responders when flaring.
1478953587 2016-11-11 Drill / Deepen / Reopen No Violations The company man on site Jack Garcia notified me by phone at 17:38 pm of a failed 5.5 casing cement job. Mr. Garcia indicated that the 8.5 well bore was drilled to a measured depth of 20,810 and 5.5 casing was ran to bottom with out issue with a flotation collar set in the string at 10,175 allowing the 5.5 casing to be loaded with fluid from 10.175 to surface. It was indicated that upon the landing of the casing internal pressure was increased and the circulation disc burst in the flotation collar and circulation established without incident. The casing cement job was executed and 549 barrels of cement pumped successfully, however during cement displacement the circulation pressure increased at 215 barrels out of a total of 438 barrels of fluid into displacement with cancellations showing the cement plug stuck in the flotation collar. The annular estimated top of cement by calculation is at 12,854 in the horizontal section with the horizontal curve being landed at 11,020 leaving a estimated 1,834 of open hole in the horizontal lateral section. The company man indicated the rig would wait on the cement to cure before demobilizing the drilling rig. Instructions were given to secure the well head assembly and install pressure gauges accordingly, the company will submit a plan to the Division before bringing in a service rig to repair the well bore.
1480948087 2016-12-05 Completion Testing No Violations Status check inspection to witness the well bore 5.5 diameter production casing cleanout. Well Service Group has a tubing snubbing unit on location and is drilling cement out of the inside of the 5.5 with cement being encountered at a measured depth of 10,165 and the company currently drilling at 16,400 going to 20,810. A bond log will be ran once the cement inside the casing is drilled out.
1495724126 2017-05-25 Completion Testing No Violations On site status check to witness the well remediation process. Upon inspection it was observed that the well is equipped with a FMC Technologies B section well head pressure rated a 10,000 psi (pound per square inch) rated working pressure. On top of the B section well head is the workover completion blow out prevention well control stack that consists of a set of pipe rams then a set of shear rams followed by a flow cross fluid release section and then another set of pipe rans topped off with a set of hydraulically controlled annular bags, all rated and pressure tested to at 15,000 psi rated working pressure. Internally the well is equipped with 5.5 diameter production casing set to a total measured depth of 20.791 and a 9.625 diameter intermediate string set to a measured depth of 9,703. The company has indicated they have stimulated 35 of 49 proposed stages to be stimulated and has stopped stimulation to repair the annular of the 5.5 diameter production casing by the means of a cement squeeze job. Key Energy has a service rig over the well and Horizontal Wireline service is Rigging up on the well to begin the preparation work of the 5.5 diameter 23 pound per foot production casing. At this point Horizontal Wireline has set a 10,000 pound rated working pressure cast iron bridge plug at a depth of 13,691 and is preparing to set a second cast iron plug for isolation purposes at a depth of 10,000. Upon completion of setting the 2nd plug 6 perforations will be shot in the 5.5 diameter casing at 10,020. After the perforations are shot in the casing Halliburton Pumping service who is also on location will attempt to establish circulation through the 5.5 diameter casing out the perforations to surface on the annular of the casing. At the point a achieved rate and pressure is established the wireline service will set a Magnum Oil Tool intervention less Cement Retainer at a depth of 9,800 and a annular squeeze job with be conducted on the 5.5 casing.

The well is in production and the master valve open. Annular pressure monitoring is present and in place. Well identification is present on each of the (2) wells present on location. Each well is selling into a gas processing unit which is located on location. Also present on location is a production fluid storage area consisting of (2) steel construction 400 barrel capacity tanks situated inside a steel containment barrier with a polymer liner.
