Halliburton Cement Service is on location to cement the casing using 330 sacks of class A cement with 25% open hole cement excess ran, the cement was displaced through the drill string stung into the cement float collar and displaced with 6.5 barrels of fresh water. The casing shall be pressure tested after the cement cures, cement returns were witnessed to surface. Also noted on this day I spoke with a Robert Bout with Stat Oils safety department in regards to the rig and support equipment not placed on containment, Mr. Bout indicated they had accessed the situation in response to my request on 04/19/2016 and on previous dates as noted in inspections under 3411124606-0000 & 3411124607-0000, Mr. Bout also indicated the company saw no issues with the product being spilt onto the earths surface and they would remediate the affected areas when the rig was done drilling, He commented that it was a drilling rig and they drilling rigs spilt things and He did not know what they wanted me to do. Photographs were taken on this day and sent to both Division Deputy Chiefs and my direct supervisor. |

The emergency shut down valve has several safety shut down buttons place on location and can also be shut down remotely through the SCADA computer system. At the rear of the well pad is (4) gas processing units and on the right (West) side of the well pad is the production facility consisting of (4) steel construction 400 barrel capacity tanks situated inside a polymer lined steel containment dike area. The entire well pad perimeter is fenced and gated with signage posted at the entrance point and on each well head. |

1003464123 | 2016-05-03 | Drill / Deepen / Reopen | No Violations | On the day of my inspection accompanied by ODNR Southeast Region District Manager Mike Moreland, Engineer Mark Atkinson and Engineering Tech Andy Thomas I witnessed Orion Drilling rig # 14 had successfully cemented the 13.375 inch intermediate casing and was performing a FIT test at 2235 feet. We met with Statoil representatives Jeremy Spires and Ben Carnes and completed walk around of the well pad. Inspection revealed that the three manual operated valves controlling the drains on the sump were all in the closed position with a 13.375 inch rubber cement plug driven into the drain pipes completely sealing them off. Oil sheen was present in the northeast sump and an eight feet by forty feet area of stressed vegetation was present outside the sediment pond serving that sump. Field tests performed revealed a pH of 6 and chloride levels ranging from 775ppm to 1082ppm. Further inspection revealed an erosion channel on the southeast corner of the well pad. Statoil was instructed to pull fluids from the sediment pond, remove contaminated soil, solidify with their drill cuttings, dispose of in an approved landfill and replace with clean fill, seed, and mulch and monitor sediment pond for chlorides. Statoil was also instructed not to release any fluids from the three sumps until no oil sheen is present and suitable chloride levels are attained. Statoil was further instructed to repair the erosion channel on the southeast corner of the well pad and determine what the contributing factor was and remediate. |

On the morning of this day I contacted the site location company man by phone at 7:43 am and was advised that during the cement displacement process on the 5.5 production long string the displacement pressure failed to indicate the cement plug had landed into the float collar and the float collar was not seating. It was initially reported that the 5.5 internal pressure was gauged at 1,100 psi with the initial cement ticket report indicating the well was shut in with 3,671 psi. The company man on site initially reported the well was bled down during the course of the next 48 hours and continued to build pressure. Upon my arrival at 14:00 hours I was then updated that the company had pumped 51 barrels of 13.8lb/gl oil based drilling mud down the 5.5 production casing with no decrees in pressure and the well bore not loading. The well was then blown down gradually over a 2.5 hour time frame and was static upon my departure. The company has submitted a proposal to run in the hole and set a cast iron bridge plug at 8,000 and load the hole with fluid on top of the plug. The drilling rig will them be removed and a tubing snubbing unit service equipment will be brought in. Also noted was the fluid that was displaced into the well bore during the cement displacement and the 51 barrels of oil based drilling mud did not flow back when the well was bled down and the company had extreme difficulty in getting the 5.5 production casing to bottom with the rig taking over 7 days to run the casing. My direct Supervisor Brent Bear has been updated of the status in 3 different conversations on this day and the company has submitted a planned proposal. |

It was also noted that on this day it was observed that a sediment pond located at EPE 15 -39.7977 X 80.90816 on the North East slope of the well pad was titrate tested for chlorides and elevated chloride levels were found to exceed a low level Quantab strip test of 611 parts per million and to be 6,171 parts per million chlorides on a high level Quantab strip tests. The sediment pond 6 outlet drain valve was leaking allowing fluids to escape. Fluids tested flowing from the drain outlet were tested and found to be the highest chloride levels of the 3 areas tested around the sediment pond. Also noted was the North East catch basin drain sump on the well pad interior was observed to have an oil sheen inside the sump and oil residue on the ground surface around the catch basin sump area. The following Directions are hereby given as to be followed-The fluid inside the sediment pond on the North -East outward slope is to be recovered and not discharged until a time when no elevated chloride levels are found. All drain valves are to be secured and remain closed. All catch basins are to be protected by oil absorbent materials and monitored regularly. The well pad underdrain and catch basin sumps are to monitored and any product found inside is to be removed. The internal working area of the well pad is to monitored and operations are to be conducted as not to allow product to contaminate the surface of the well pad and permeate the soil.
