0100176108 2017-12-22 Casing String Witnessed No Violations Onsite inspection to witness the conductor casing cement job with Express Energy Services. The rig drilled a 26-inch diameter borehole to a depth of 100 feet from ground level. 100 feet of 20-inch diameter weld connection casing was inserted into the borehole. At approximately 24 barrels (114 sacks) of cement into the job, the conductor became plugged and was unable to continue pumping cement. Attempts were made to lift the conductor without success. The job was completed by filling the annulus of the hole with 153 sacks of Class A cement with a 1.18 yield, 2 percent calcium chloride, 0.5 pound of flake, and witnessing returns to surface. The cement proceeded to slowly settle back into the borehole. The cement appears to be being lost to a formation approximately 40 feet in depth downhole. They are going to let it settle and solidify for a few days, and come back to top it off. The remaining cement left in the conductor will be drilled down to 90 feet, thus leaving a 10-foot shoe in the conductor.
0827917438 2019-04-16 Preliminary Restoration No Violations On this day, I am conducting a preliminary restoration inspection. The site meets ORC 1509.072 (A). Annular pressure monitoring devices are equipped, along with proper identification. The 9.625 inch diameter intermediate casing has a pressure of 250 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 0 psi. The production records are current.
0863890212 2018-02-20 Drill / Deepen / Reopen No Violations On this day, I am checking in with CNX Company Man Kevin Perdue while the Nabors X13 rig is in the process of rigging up. Kevin has stated that they expect to start drilling February 23rd. I conducted a perimeter walk of the pad which appears to have drainage issues and considerable amounts of standing water across the surface. Due to the excessive rainfall, removal of the water will be addressed.

Please reference his comments for a detailed report. Nabors X13 drilling rig is still in the process of rigging up and anticipated to start drilling on February 23rd. Due to the new crew on this location, contact information was exchanged. Discussions about the removal or discharge of the rainwater on the pad were addressed.
1514299862 2017-12-26 Casing String Witnessed No Violations Onsite inspection to witness the conductor casing cement job with Express Energy Services and Caldwell Redi-Mix. The conductor was topped off with redi mix concrete by dumping it down the annulus of the hole and filling it to surface. Cement is witnessed at surface and has stabilized at the cellar floor.
1519334559 2018-02-22 Drill / Deepen / Reopen No Violations Status check inspection and pre spud meeting accompanied by ODNR Inspector Tom Thomas. On this day we met with Stat Oil representatives Colin Heckler and Mike Cannon and discussed the well pad containment and rig under containment and well as the drilling permit conditions and casing program. Neighbors rig X 13 is set op on location and is scheduled to spud in the next 48 hours. The Stat Oil Representatives have been provided with Inspector Tom Thomas contact information and understand He will act as the lead point for the Division. The well pad operations regarding containment have been explained and the Company understands the Division has no set directives regarding direct operations and only if there is product released in excess or above reportable quantities will we give direction.
1519708535 2018-02-26 Casing String Witnessed No Violations On this day, I am onsite to witness the 13.375-inch diameter surface casing cement job with Halliburton Energy Services. The Nabors X13 rig drilled a 17.5-inch diameter borehole to a depth of 1,852 feet from a KB of 32 feet. Wyoming Casing inserted 1800 feet of 13.375-inch diameter J-55 54.50 lb/ft casing into the borehole. Halliburton Energy Services initially pumped 40 barrels of 6% gel spacer, followed by 10 barrels of fresh water spacer. They cemented the casing with 348 barrels total volume of 1,595 sacks of Class A cement with a 1.227 yield, and a weight of 15.8 ppg gallon. It was displaced with 270 barrels of fresh water. The plug landed with 750 psi and then pressured up to 1,250 psi. 30 barrels of cement returns were brought to surface.
1520532740 2018-03-08 Drill / Deepen / Reopen No Violations On this day, I spoke with company man Kevin Perdue, and found the Nabors X13 rig has drilled the 12.25 inch diameter borehole to a total depth of 9, 843 feet. Wyoming Casing is currently running 9.625 inch diameter casing in the hole. They are projected to continue drilling with oil based drilling fluid. It will be noted that the Clinton formation top came in at 8,604 TVD.

1659350763 2019-01-28 Production Wells No Violations On this day, I found the pad gated with signage at the entrance. The drilling and completions processes are finalized and the wells are now in production. There are (3) producing wells, (3) gas processing units with sand-traps, (3) 400-barrel steel production tanks within steel containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Concrete barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 600 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 300 psi. It will be noted that I spoke with Equinor representatives Jarrett Howell, Pat Reynolds, and Will Cramblett during the inspection. Additionally, EnerCorp is onsite with secondary sand-traps present at the wellheads.
