0209264248 2018-09-24 Casing String Witnessed No Violations On this day, I am onsite to witness the 13.375-inch diameter surface casing cement job with Halliburton Energy Services. The Nabors X13 rig drilled a 17.5-inch diameter borehole to a depth of 1,920 feet from a Kelly Bushing of 32 feet. Nabors Drilling used their CRT and inserted 1,895 feet of 13.375-inch diameter J-55 54.50 lb./ft. casing into the wellbore. A pre-job safety meeting was held in the doghouse. Halliburton Energy Services initially pumped 40 barrels of gel spacer, followed by 10 barrels of fresh water. They cemented the casing with 347 barrels total volume (1590 sacks) of Class A NeoCem cement with a 1.227 yield, and a weight of 15.8 pounds per gallon, verified by scale. It was displaced with 286 barrels of water. The plug landed at 1,000 psi and pressured up to 1,500 psi, and held for 5 minutes. The floats held and bled back 2 barrels. 101 barrels of cement returns were witnessed at the surface.
0469434728 2018-10-09 Drill / Deepen / Reopen No Violations On this day, I am accompanied by ODNR Inspector Jeff Adams. I found the Nabors X13 rig currently drilling 8.5 inch diameter production borehole with oil based drilling fluid at a measured depth of 14,150 feet with an azimuth of 138.88 degrees and an inclination of 88.88 degrees. The projected total measured depth of the well is 21,494 feet. I spoke with Company Man, Colin Hecker, during the inspection. A perimeter walk was conducted, and found all drain valves in the closed position. Truckloads of 5.5 inch diameter production casing are currently being unloaded on the pipe racks.
0926361948 2018-05-09 Casing String Witnessed No Violations Onsite inspection to witness the conductor casing cement job with Express Energy Services. On 05/08/2018 the rig drilled a 26-inch diameter borehole to a depth of 120 feet from ground level. Then, 120 feet of 20-inch diameter (I.D. = 19.124) weld connection casing was inserted into the borehole. Then, 7 barrels of fresh water were pumped ahead of cement. Approximately 4 barrels into cement, the pump truck circulator became plugged and unable to continue the job. 60 barrels of fresh water were pumped down the inside of the conductor to clear it of cement, and brought it back to surface. The job will be performed at a later date.

On 05/08/2018 the rig drilled a 26-inch diameter borehole to a depth of 120 feet from ground level. Then, 120 feet of 20-inch diameter (I.D. = 19.124) weld connection casing was inserted into the borehole. Then, 6 barrels of fresh water were pumped ahead of cement. They cemented the casing with 48 barrels total volume of 230 sacks of Class A cement with a 1.18 yield, 2 percent calcium, 0.5 pound of poly-flake per sack, and displaced with 39 barrels of fresh water. Cement returns witnessed at surface.
1538740066 2018-10-04 Drill / Deepen / Reopen No Violations On this day, I found the Nabors X13 drilling rig connecting their CRT and preparing to run the 9.625 inch diameter casing. It is projected to be cemented within the next 24-48 hours. I spoke with Company Man Colin Hecker, during the inspection, and he indicated that they will be using the DV Tool within the casing string. The rig drilled the 12.25 inch diameter borehole to a measured depth of 10,243 feet.
1553729048 2019-06-11 Completion Testing No Violations Status check inspection found the well drilling and completion process complete and a snubbing unit style completion rig owned by Deep Well is on location and rigged up on the said well. The rig # is Snub 14 and the rig is running in the hole with a workover string to drill out the completion plugs. The well bore is being equalized with 9.3 pond production fluid. The 2 primary master valve on the completion stack are owned by FMC well head. 5 wells were recently drilled and completed.

They are zipper fracing between permit numbers 4829 (SU2H) and 4827 (SU2H). I spoke with Equinor Company Man Loren Blake, and Darren Galisky during the inspection. They are currently fracing stage 29 of 49 with 30 shots, divided into 3 clusters. The frac plug is set at a measured depth of 15,688 feet. They are projected to commence completion operations on permit number 4818 (M5H) within the next 24 hours.

I am conducting an inspection during the completion process, and found Halliburton Pressure Pumping Service onsite along with Allied Horizontal Wireline. They are zipper fracing between permit numbers 4818 (M5H) and 4827 (SU2H). I spoke with Equinor Completions Superintendent Daren Galisky during the inspection. They are currently perforating stage 32 of 49 with 30 perforations. The frac plug is set at a measured depth of 15,087 feet.
2040928377 2019-05-15 Completion Testing No Violations On this day, I found the hydraulic fracturing process has been completed, and the crew is currently demobilizing and leaving the pad. I spoke with Equinor Company Man Tyler Sanregret during the inspection. He stated that Deep Well will be rigging up a standalone snubbing unit to drill out the frac plugs, commencing the week of 5/20/19.
