0157742270 2017-01-27 Plug / Plug Back No Violations On site inspection to witness the well Bond Logging process by Appalachian Wireline Service with Harmons Well Service rig on the hole. The 2 production tubing has been removed and the 4.5 diameter production casing loaded with water in attempt to run a bond log and casing collar locator. Appalachian Wireline ran in the hole showing a total depth of 3,750. Perforations were found between 3,734 to 3,738 and 3,678 to 3,710 ,the Berea was also perforated between 1,670 and 1,680. Historic records indicate the well was initially completed as a shot hole completed in the Keener sand from 1,103 to 1,138 and the bond log tool showed no cement from 1,300 up and a shot hole below 1,100. The 4.5 diameter casing would not hold a constant hydrostatic column of fluid past the Berea Sand. The plugging contractor Harmons well service will set the 2 bottom cement plugs then pull the 2 plugging string and at that point determine if the 4.5 casing will hold a hydrostatic column of fluid internally, if the well bore loads with fluid another bond log will be ran to determine an exact top of cement and the casing will be severed with a electric detonated casing cutter and the well plugged through the 4.5 to surface. The original 6.625diameter casing string is in the hole and records indicate it was set to 1,113 with the bond log showing activity at that level.
0243596430 2014-07-24 Production Wells Violations Noted Status check inspection per request of the landowner as part of complaint # 4753 filed by the landowner. On the day of my inspection the following was found present at the well head 7 casing-3.5 casing - 2 production tubing and pumping rods with the production valves in the open position and the well set up to be produced by the means of a pumping unit with dense vegetation covering the entire back half of the pumping unit and multiple production tanks in the area with no distinctive tank found in service or related to the well, no well identification was posted and no gas metering equipment was found related to the said well. Database records shows the reported production not meeting the minimum requirements of 100 mcf per year since 2012 and scatter production prior to 2012. The said well shall be plugged within 180 days. ODNR Monroe County inspector Bill Stolarik 740-507-6592.
0528825429 2017-01-30 Plug / Plug Back No Violations On location to witness the bottom plugs to be set with Harmons Well Service rig on the hole and Formation Cementing. The rig had previously pulled the 2 diameter 10 round tubing and pumping rods and on this day tallied the tubing and ran it back in the hole with 3,720 of tubing installed back in the well. Also noted was a wireline loggers total depth on 01/27/2017 of 3,750. Formation Cementing pumped the bottom plug by first pumping 19 barrels of 6% gel spacer then a 28 sack 400 plug displaced with 10.2 barrels of fresh water. The tubing was then pulled up to 1,740 and a 250 20 sack plug set to cover the Berea Sand formation perforations at 1,671 to 1,679. The plug will be tagged by the wireline crew before cutting the 4.5 on a proceeding day.

1063931949 2016-04-12 Production Wells No Violations On this day I received phone notification from Tyson Brock with B&N Coal who indicated they would be moving in to plug the said well. He question the approved plugging method allowed and informed me the well had been drilled with a rotary machine to past 4,000. Mr. Brock was informed that cement would be the required plugging method to be spotted through tubing and any plugs or obstructions in the well bore are required to be removed.
1425516717 2015-03-04 Production Wells No Violations Compliance notice # 243596430 and complaint # 4753 follow up inspection. Upon inspection of the well head it was observed that the 1 tubing flow line was disconnected and production casing flow line was connected at the well head but no gas meter of sales point can be identified. The pumping unit is still in disrepair with no change in the mechanical aspect of the unit from my last inspection on 07/24/2014. The initial violation issued on 08/28/2014 has not been responded to by the producer.
1475083116 2016-09-28 Production Wells No Violations Compliance notice follow up inspection found the well equipped with 4.5 production casing - 2.375 production tubing and pumping rods with no electric service present to power the drive motor and no flow lines hooked to the well head.
1479333282 2016-11-16 Production Wells No Violations Compliance notice # 243596430 and complaint # 4753 follow up inspection accompanied by Division Supervisor Brent Bear and Ohio Attorney General Scott Myers. Upon inspection of the well head the well was observed to equipped with 4.5 production casing and 2.375 production tubing along with a pumping tee installed on the tubing and pumping rods inserted in the well bore. The well is set up to be produced by the means of a pumping unit with no production flow line attached to the pumping tee. It was also observed that the dense vegetation has now been removed from around the well and a 1 gas sales line could be seen on the backside of the production casing, the line was followed and appeared to feed a nearby mechanical separator and then lead to a residential structure across the road. The resident indicated the well was providing him gas but he did not own the property the well was on. The previous violation shall be considered resolved on this day.
1485526076 2017-01-26 Plug / Plug Back No Violations Status check inspection on the well plugging process found Harmons Wel Service on location pulling the 2 diameter 10 round tubing.
1485983794 2017-02-01 Plug / Plug Back No Violations Appalachian Wireline Service was on location and ran in the hole to tag the top of the Berea plug and find the top of the cement behind the 4.5, the top of cement on the previous plug was at 1,534 and the 4.5 was attempted to be cut off at 1,272 and did not cut, a 2nd attempt to cut the casing at 1,145 was successful, then one 4.5 diameter casing joint pulled up to 1,128 and a 400 plug was set to isolate the bottom of the 6.625. A 300 surface plug was then set and the remainder of the 4.5 recovered out of the hole.
1486398757 2017-02-06 Plug / Plug Back No Violations Status check inspection to witness the grouting of the annular space between the 6.625 and 10 conductor. On 02/03/2017 I was contacted by Troy Harmon who indicated that while digging down to cut the 6.625 after plugging the said well a 10 conductor string was encountered. I notified my supervisor Brent Bear and the decision was made to grout the annular space through a 1 tubing string. Harmons Well Service ran a 1 tubing string down the annular space to 200 and grouted the void on this day with 72 sack with class A 1% calcium 1% gel cement with cement witnessed to surface.
1702604154 2017-01-25 Plug / Plug Back No Violations Status check inspection on the well plugging process found Harmons Well Service rig on the hole pulling the .625 diameter pumping rods out of the hole. They then proceeded to pull the 2 diameter 10 round thread production tubing. The 4.5 diameter production casing is pulling stretch with the tubing indicating a low cement top. Once the tubing is pulled the well will be logged and plugged accordingly.
2007086052 2017-01-10 Plug / Plug Back No Violations Administrative Inspection to document the conversation I had on this day with Mike Kendall with B&N Coal regarding the up coming well plugging procedure. Mr. Kendall indicated the company had no records on the well completion process. Mr. Kendall and myself were able to determine through the Division Database records that the well did appear to be deepened to the Oriskany and plugged back to 3,800 on 04/12/1968 with 4.5 production casing being set and cemented with 300 sacks of cement. Mr. Kendall was informed that they will need to set a 400 bottom plug and a 200 plug to cover the Berea. I also informed Mr. Kendall that the 4.5 would need to be severed and a rip plug set and the old shot hole filled if present and the 6.250 isolated top and bottom. I suggested the log the well and find the top of cement and determine if the original shot hole was still present or had been cemented over prior to deepening the well or during the completion process. Mr. Kendall understands will with further evaluate the process once a log is in place.
