0052174281 2019-04-10 Production Wells No Violations The inspection is due to a Compliance Notice follow-up to number -1668657151 and complaint number 5839. Jason Blocker the Emergency response officer of the Division and I looked at the tank site and the well. The last time we were on site was back in 2-12-19 and it was raining. The inspection revealed that it had dried up and it is time to remove the small dams in the ditch below the tanks. Remove the yellow bucket and add a flex pipe and direct the water down to the ditch from the cavern. Remove all debris and equipment from the clean-up. Upon inspection of the well and the last inspection in 2-12-19 and talking with the well tender Tracey Lindamood the 2-inch flowline is not fixed and got an H2S reading of 60.4 ppm on the Tango meter. The line has a hole in the side and is blowing in the atmosphere. There will need to be a burner stack installed due to the non- sales of the gas. The company shall have the line fixed within 5 days and install the bull plug within 5 days.

Jason Blocker and I looked at the site and revealed that the equipment has been moved off the tanks site. There has been secondary containment in place. There was a second coffer dam installed.
0283915677 2018-08-01 Production Wells No Violations The inspection was conducted under a Compliance Notice Follow-up to number -1668657151. The well is equipped with a pumping unit and electric motor, tubing head, one of the two tubing valves are open, and one of the two casing valves are open. The tank site is back intact and consists of [1]210-barrel steel tank and [1] 50-barrel steel tank. The tanks are enclosed within an earthen impoundment and plastic liner is installed below the tanks. There is a dike drain with bull plug due to impoundment holding water. The vegetation is starting to come in but at the bottom of the bank there is still crude oil coming from the hill side. The rains are purging the oil out and will monitor the two cofferdams placed in the ditch to catch the oil. The well tender Tracey Lindamood will maintain the absorbent pads in the cofferdams.
0858626786 2018-05-11 Production Wells No Violations The inspection was conducted as a Compliance Notice Follow-up to number -1668657151 and complaint number 5839. The location was dry and clear and the current temperature is at 80 degrees Fahrenheit. Zemba Bros. are continuing the cleanup. Zemba is flushing the ditch next to the road and extracting the oil saturated leaves and material out with a dump truck and a track-hoe. The Ohio Environmental Protection Agency (OEPA) Chris Holmes is on site. The creek has trace amounts of oil in creek. Mark Depew the owner of Petrox Inc. and his pumper Tracey Lindamood were on site. Tracey had shut the well in and I was explaining to Mark Depew that the tanks will need to come out and the earthen impoundment will need dug out and all contamination will need hauled away. He wants to monitor the site. There were some more coffer dams installed to let the water through and retain the oil. The road is currently closed to traffic. I will check back at a later date.
0979310504 2018-06-15 Production Wells No Violations The inspection was conducted under dry, hot humid conditions. The temperature is 74 degrees Fahrenheit. The inspection is a Compliance Notice follow-up to number -1668657151 and Complaint number 5839. The tank site was inspected only on this day and revealed the equipment has been delivered to complete the job of setting the tanks back in place. The impoundment will have to be east of the drain. The drain will allow the hillside to flush and be separate from the earthen impoundment. The [1]210-barrel steel tank and [1]50-barrel steel tank were on bank. There are 2 coffer dams at the road that are aiding in the purging of the crude oil. I spoke with Gary Sheets and they would be finishing up on 6-18-18. I took pictures on the day of the inspection.

Zemba Bros. company is done with the preliminary work. I called Mark Depew of Petrox and stated to him that the initial monitoring is not cleaning up the crude. The crude is still coming out of the bank where the tank site is located. Mark Depew of Petrox called Gary Sheets a local excavator that cleans up oil spills. They will be planning to move the tanks and start hauling material out.
1526925779 2018-05-15 Production Wells No Violations The inspection is a follow-up to compliance notice number -1668657151 and new complaint 5844. The area is under clean-up operations after a tank spill. The landowner Brian Dillon on 6515 Buttermilk road stated that the sour gas smell is getting to gaseous and overwhelming. I went to the area and took some readings and there was no LEL on the Ventis pro 4 gas meter. I called the company and they would pay to place people in Hotel until the smell resides. They talked to the company and declined.
1527003771 2018-05-22 Production Wells No Violations The inspection is conducted under raining, humid conditions and 68 degrees Fahrenheit. The inspection was conducted as a Compliance Notice Follow-up to number -1668657151 and complaint number 5839. The inspection notes the following: The tank site cleanup resumes with the large hole in the rock located within the earthen impoundment. Currently there is none at this site working. The oil is draining out with the water that is exiting the large hole and is trapped within and absorbent boom and diapers. The area will need monitored and the absorbent diapers and booms will need changed out as needed. The oil is seeping out of the ground down below the equipment and is caught in the absorbent diapers with in the double siphon dam. There may be a need to install a solid drain line and seal the hole off, then construct the tank impoundment just east the drain pipe. The installation of the drain pipe will relieve the pressure on the area.
1555422689 2019-04-16 Production Wells No Violations The inspection is a compliance notice follow-up to number -1668657151 and complaints 5839 and 5844 respectively. I stopped in to look at the well and it is still venting gas at 60 ppm of H2S. I called and talked to Bill McGraw of Petrox on 4-16-19 at 9:31. He is coming in to the well in morning and installing the flare stack. He stated that a truck will be in and clean up at the tank site.

The tubing valve is open to the tank site and the casing valve is open to the flowline leaking and is venting to atmosphere. I was getting H2S reading of 8 on my side from the Ventis pro 4 gas meter. I called Petrox (Bill McGraw) and he stated that pumper is in the area. Tracey Lindamode the well tender and I met, and I stated that the line needs repaired, and needs hooked up and flared. The impoundments will come out as soon as the weather dries up.
