0229065639 2015-03-16 Plug / Plug Back No Violations Construction began on the access road to the well from the county road. This required cutting the fence to make a 30 wide opening and then applying gravel. The permit required a pad of stone 30 wide and 100 long which brings it half way out in the pasture to the well. This gravel access is a requirement of the county but the landowner may ask to have the gravel removed upon completion of the plugging. Four 18-ton loads of gravel were brought and spread and the remainder will be hauled tomorrow. I spoke with the County Inspector over the access construction and he said the purpose was to prevent mud from being drug out on the road and could not give us a waiver. The container for contaminated soil, dozer, and larger track hoe were brought and placed on location and the rig was still to come this day. The smaller track hoe dug around the wellbore and determined that the contaminated soil was no deeper than 4-6.
0276780817 2015-05-18 Final Restoration No Violations My inspection today finds the well site has a thin covering of vegetation; however, the gate has not been installed to satisfy the final restoration conditions. I called Gene Chini and notified him that work was not complete.
0356025665 2015-03-17 Plug / Plug Back No Violations Two more loads of gravel arrived for the access road to the well and it was completed as required. Welding was done to prepare the casing for the wellhead. The contaminated soil was cleaned out from around the well bore and loaded into a steel half tank. The cellar was installed and sealed in the bottom with 6 sacks of cement. The crew rigged up over the hole and late in the day, ran in the hole with a 3.5 bailer, getting to 609 and recovering fire clay. Two more runs recovered fire clay to a depth of 635. The will come back tomorrow with a bigger bailer and drill stem, jars and a bit to try to get deeper.
0649220997 2015-06-15 Plug / Plug Back No Violations My site inspection found that the location and surrounding area has been disked up, seeded and mulched with straw. It is not obvious to me that they brought in additional topsoil, but the ground has been smoothed and the seed is starting to sprout. I will not be able to sign off on this restoration work today as there is still equipment laying in the field (a large bailer, a roll of plastic and a section of 2 piping) and the fence has not been fixed and the gate not installed.
1307038316 2015-03-18 Plug / Plug Back No Violations Work began today at noon with delivery of the larger bailer and more welding on the casing. Charlie, the rig operator, went in the hole with the 5.5 bailer and bailed to bottom. The bailer brought up fire clay from 609 to 635. At 635 , they hit solid material and the drill bit, stem and jars were added to try drilling out this plug. This more solid sandy gray material was drilled to 644 when they encountered something more solid. They built an impression block to run in the hole to try to get a picture of what was blocking the hole. It brought up no results but will be run again in the morning.
1336655585 2012-05-10 Orphan No Violations Inspection of well found 7 casing only ,well is leaking from behind the 7 RBDMS records show surface was mudded in and plugging will have to consist of isolating annular of 7, There is a affected kill area of approximately a 30 x 30 area around well, The well was drilled in 1983 but no production records on file. The well is referred to Idle Orphan status due to producer having lost bond to forfeiture.
1394739080 2014-03-12 Orphan No Violations Talked with landowner, Judith Anderson, and she said they had a water well but had not used it in years - was now on Pure water. The water well was located under the concrete slab next to the house and there is no access to it. She had no information on how deep it was but said it was only used to water the cattle after they got the rural water. GPS location is N39.72449, W 81.51427 and it is not in the ODNR database.
1401289964 2014-05-27 Administrative Inspection No Violations I talked with Charlie Pavlik late in the day and he had gone out and dug around casing with a shovel, finding that there is surface casing and conductor casing in the well. This should make it simpler to plug and he should put the wellhead control device on the surface casing. He also talked to the landowner while out there and he suggested we put a gate in the corner of the fence around the pasture to make entrance easier and less destructive and I gave permission to do that, as long as Ms. Anderson agreed with it and he said she had.
1427123640 2015-03-20 Plug / Plug Back No Violations The 8 5/8 casing was tested to see if it would pull. It came out easily, due in part to the fire clay not remaining in place when the pipe was originally set and falling to the bottom of the hole. It was decided to pull the 8 5/8 casing and leave the 7 for surface casing. The joints were cut with a torch as they came out and tallied at 252.
1427297433 2015-03-23 Plug / Plug Back No Violations Today the rig crew ran 2 3/8 tubing in the hole to 628. Formation Cementing set a bottom plug of 300 using 68 sacks of 15.6#/gallon Class A cement. The rig pulled all tubing and JET Wireline logged the well to verify it had casing, then shot perforations at 200 to squeeze cement into. The rig went in with 300 of 2 3/8 tubing and the cementers set a 100 plug using 30 sacks of cement. All tubing was pulled and the cementers connected to the 7 casing in the well, pumping 133 sacks of cement inside the 7 and outside the 7. Good returns and the cement stayed at surface.
1427297799 2015-03-24 Plug / Plug Back No Violations Today was spent moving the equipment off location and cleaning up the site with the track hoe. All that remains on site is a steel half tank filled with contaminated soil. At 4:30, Charlie cut off the casings and the backside of the 7 casing was topped off with 4 sacks of cement mixed with water. The steel plate with identification was tack welded over the hole and it was covered with soil. The man renting the pasture stopped by and verified that the gravel used for an access into the pasture could remain where it was. Four hours of this time was spent doing clerical.
1435591866 2015-06-29 Final Restoration No Violations I inspected this well site and found the bailer and all other equipment has been removed, the entry gate is now up, vegative cover is established and the landowner is satisfied and has signed the Landowner Waiver. Final Restoration is passed.
1774158569 2014-03-07 Orphan No Violations I re-inspected this well to gather additional information and found this well to be 250 from the banks of the West Fork Duck Creek. I noted oil and gas is bubbling up from the outside of the 7 casing which was reported to be cemented in place. There was no oil noted in the stream at the time of my inspection. It is apparent from the oil impact on the ground around the well that a severe rain event could cause impact to the stream. This well has been moved up to emergency status and will be submitted for review by the Coast Guard. New SEM is 227. More pictures taken.
2009924859 2015-03-19 Plug / Plug Back No Violations Work began this day by running the 5.5 bailer in 3 times to clean up the hole. Then the impression block was run in to try to get a better impression of what was blocking the hole. The results were inconclusive. Based on the fact that the well is not making any oil or gas and that total depth was not known, it was decided to plug from this point of 644 to surface. The rest of the day was spent getting the casing ready for the wellhead and placing the 13 valve on the conductor pipe. Additional tubing that was needed to get to the bottom of the well was delivered.
2066607579 2015-06-01 Plug / Plug Back No Violations This inspection is required to pass the reclamation work and I find the work has still not been completed. There is still a bailer and a 100 roll of plastic laying in the grass. There are still several spots that need to be seeded and mulched, but the soil type is primarily clay and water ponds in this area much of the year. The gate and an extra support post for the gate still needs installed where the fence was cut for entry into the pasture.
