The engineering group is monitoring the slip. A geotech drilling rig recently drilled a test hole on the well pad to test for stability. The upper fresh water impoundment has been emptied due to a large slip.

The pad consists of 8 producing wells. The tank battery sits on the left side of the pad as you enter it and consists of (8) 400 barrel tanks. Identification is present on each well head. The pipeline tie in was located on the West side of the pad. The identification was located at the wellhead and the production records were current.
0986711054 2013-12-21 Casing String Witnessed No Violations My inspection today finds the drilling rig, Falcon 20, ran 419 of 18.625 casing. The pre-cementing test of the mix water found no chlorides. Allied Well Services cemented the casing in place with class A cement.

1069355868 2019-01-31 Production Wells No Violations On the day of my inspection accompanied by ODNR inspector Bill Stolarik I witnessed that the well was equipped with 5.5-inch diameter production casing and 2.875-inch diameter tubing utilizing a plunger lift system as means of production. At the time of my inspection annular pressure monitoring was in place on the 9.625-inch diameter deep intermediate casing, and 13.375-inch diameter intermediate casing. Production equipment included a Gas Processing Unit, as well as 8- 400-barrel steel production fluid storage tanks, and a combustor that was shared with the other producing wells on the pad. There was secondary containment around the production fluid tanks, and the Gas Processing Units. The well pad was contained within an earthen dike, there were multiple Emergency Shutoff Devices on location. There was a midstream valve site on the west side of the pad. As well as a water impoundment to the south, right side of the lease road as you enter the pad. Identification tags were present, and production was up to date.
1403109571 2014-06-17 Casing String Witnessed No Violations I am onsite to witness cementing operations of the production casing string. The drilling rig, Patterson 332, ran 17563 of 5.5 casing. Allied Oil and Gas Services circulated a 341 sack 50/50 POZ lead cement and 1620 sacks of class H tail cement.

The tank battery consists of eight 400 barrel tanks set inside of a plastic lined steel containment system. On the opposite side of the pad there are eight production units, also set in containment. Several pieces of construction equipment are parked on the location and are being used for the reclamation of the adjacent fresh water impoundment.

1999681125 2013-11-21 Casing String Witnessed No Violations My inspection today finds the drilling contractor, Express Energy Services, drilled 100 of 30 hole. The rig then ran 100 of 26 conductor. Express cemented it in place with 330 sks of class A 2% CaCl cement. 25 bbl of returns were witnessed.

Antero Resources was on location preforming maintenance on a gas production unit.
2075842585 2013-12-20 Casing String Witnessed No Violations On site to witness surface casing job, Falcon Drilling Rig#20 they drilled a 24 hole to 420 and were proposed to run 420 of 18.625 casing I witnessed them run two joints of casing and then they were having trouble getting pipe to make up and it was taking them several minutes to get to next joint of casing I told rig consultant to call when they were ready to cement pipe in place.

Note: Allied cementing on site to cement pipe in place.
