0232570943 2017-11-01 Plug / Plug Back No Violations I stopped by this well today to check on the clay. The plugging clay is not at the surface at this time. I contacted Zack Poling to let him know the clay had settled again. He said he would contact Kenny Wright to find out why he has not finished this yet. He was under the understanding that the well was finished.
0380243732 2017-09-07 Plug / Plug Back No Violations I have returned to this well today to witness the plugging operations on it. The rig crew has started to wash in the bottom plug using plugging clay from Logan Clay. After getting in around 1 ton the fluid started coming out of the 8 inch casing so we had to shut down. With the hole being that high in the 4.5 inch casing we decided the best thing would be for them to run tubing back in the well and set the bottom plug with cement.
0644914819 2017-09-08 Plug / Plug Back No Violations I returned to this well today to witness the plugging operations on it. When the rig crew was running the tubing back in the well yesterday for the bottom plug they ended up sitting down at 1800 feet. So this morning Huffman and Bowers has there pump truck out on location to wash the plugging clay out of the well so they can get the tubing to bottom. After we got the hole cleaned out and the tubing to the bottom a 30sack cement plug was set using Class A cement. The tubing will now be pulled from the well so they can rip the 4.5 inch diameter casing. The tubing will be ran back in the well after they pull out a couple hundred feet of 4 inch so they can set the Lime plug through it.

0660776364 2018-07-24 Plug / Plug Back No Violations I was back on location today to check the well to see if it has been filled back to the surface with plugging clay. At this time the clay is at the surface and the well is ready to be cut off so they can start reclaiming. I will make a phone call to Zack Poling to let him know.
0813826649 2015-01-15 Production Wells No Violations During the field inspection of this well I found it appears to be capable of production. There is a pumping unit on the well with rods and 2 tubing down hole. The valves coming off of the wellhead to the production tank are open so the fluid can flow to the tank. The identification for this well is located at the tank pad. There are no violations to report at this time.
0972069105 2017-09-14 Plug / Plug Back No Violations I returned to this well this morning to see if the plugging clay was at the surface. At this time fluid level is down about 75 feet. I am not sure on the depth of the clay. The rig crew washed in more clay up to the surface. We will leave it open for a few more days so I can check it again before it is covered up.
1003485815 2017-09-06 Plug / Plug Back No Violations I was on location today with Dan Goins, Eric Conrad and Joe Hoerst for some training. Kenny Wright has moved his service rig in on this well to plug it for Mr. Poling. At this time they just finished pulling out the rods and tubing from the well. They are now setting up there 4 inch tools to run them to bottom to check the depth and make sure they can get down to the bottom. They believe there is a hole in the 4 inch casing about 150 feet down from the surface.
1142097846 2016-06-03 Production Wells No Violations This well is capable of production. The well has a pumping unit on it with rods and 2 tubing in it. The valves are open from the well to the tank pad. The identification for the well is located on the production tank. There are no violations at this time.
1218758602 2018-05-07 Plug / Plug Back No Violations I have returned to this well to check the status of it. The well is not plugged to the surface at this time. The fluid level in the hole is around 30 feet at this time but I can not tell where the clay level is at. I will contact Zack Poling to let him know the clay settled if it was topped off again and that the 4.5 inch diameter casing is still laying on the ground.
1292418416 2017-10-04 Plug / Plug Back No Violations I returned to this well today to check to see if the clay was at the surface. At this time the plug is down at least 150 feet from the surface. I contacted Zack Poling to let him know that the well is not complete. He said he will get ahold of Kenny Wright and send him back out to finish the job.
1505298750 2017-09-12 Plug / Plug Back No Violations I returned to this well today to check on the plugging operations. Today the rig crew set the 7 inch pre-cast plug at a depth of 1,025 feet. They then washed is some plugging clay before working on retrieving the 8 inch surface casing. They were unable to retrieve any on the 8 inch casing. There was no stretch on it at all and we could not get any telegraph. The well will be filled with plugging clay from the pre-cast plug to the surface. We will leave the well open for a few days to make sure it does not settle.

1766249184 2017-09-05 Plug / Plug Back No Violations I received a phone call from Kenny Wright letting me know he has moved in on this well to plug it for R.C Poling. I stopped by the well to see how he was getting along. At this time the rods and tubing are still in the hole. The tubing is parted about 100 feet down. They are getting a fishing tool right now to try and retrieve the tubing.
2006870296 2017-10-11 Plug / Plug Back No Violations I returned to this well today to check on the status of plugging this well. The clay has settled down the hole at 30-40 feet. I talked with Kenny Wright and he will be back on Sunday to top the well off again.
