0101473169 2005-02-01 Production Wells Violations Noted Wade Pol Contracting personnel and equipment were on site removing impacted material inside of the tank battery. The material was transported to Sanders Water Service yard and placed on plastic. Final disposal has not been decided. After the impacted material was removed, the exposed native clay was covered with a layer of Peatsorb and the tank battery was restoned. The tanks will be reset on 02/02.

1965039582 2005-01-31 Production Wells Violations Noted John Fleming with Great Lakes Energy called and reported a tank overfill at the Roosa #1 tank battery. The loss was estimated at 5 bbl when the pump jack timer failed and the pump jack kept running. The release was contained within the tank battery. Wade Pol Contracting personnel were on site with two water trucks. One for washing with hot water and the other for recovery. After seeing the extent of impact, I requested that Great Lakes empty and remove the 100 bbl tank and remove the impacted stone inside the tank battery. John Fleming agreed and the work was scheduled for the next day.
