0320390884 2017-11-29 Final Restoration No Violations I was on location to check the progress of Final Restoration at this recently plugged and abandoned well location. The well was plugged on 11/10/17 and Final Restoration is due by mid February (3 months). The wellsite is graded, the flow line has been pulled, and all surface equipment has been removed. A yellow, wooden post has been placed at the location of the former wellhead as this area is going to be developed for commercial use. FR approval is pending the receipt of the Landowner Waiver (Form 5). No violations.
1349670260 2017-11-09 Plug / Plug Back No Violations I was on location to witness plugging operations at this Clinton Sandstone well. Plugs were set across the top of the Big Lime and across the Berea Sandstone/ Ohio Shale interval. The casing was recovered and the remainder of open surface casing will be bailed dry and grouted to surface tomorrow (11/10/17). No violations. See plugging report for more details on this job.
1414770885 2015-10-08 UrbanProduction Wells No Violations I was on location for a status check. During my inspection, I found the well in production. The well is equipped with an electric powered pumpjack. The tank battery consists of a 100 barrel production tank, a buried, concrete, brine tank, and a mechanical separator. There is a dike around the tank battery and well. An identification sign is posted. Locked, chain link fences surround the tank battery and pumpjack. The lease road is in stable condition. The well was placed into production prior to House Bill 278.
1509986630 2017-11-06 Plug / Plug Back No Violations I was on location to inspect this Clinton Sandstone well prior to plugging and abandonement operations. The wellhead consists of 4.5 production casing, 2.375 tubing and rods, and a stuffing box. The pumping unit has been removed and the flow lines are disconnected. Bishop Well Service is scheduled to be on location this afternoon. The tank battery, tanks, and surface equipment have been removed. I will check back later to see if the service rig has moved in. No violations.
1510162071 2017-11-08 Plug / Plug Back No Violations I was on location to witness plugging operations at this Clinton Sandstone well. Bishop Well Service was the plugging contractor and Petroset cementing was the cementing contractor. A 7.3 barrel (30 sack) plug was set across the Clinton Sandstone with 2.375 tubing. The tubing was pulled and the 4.5 production casing was ripped at a depth of 2586. The wellhead was packed off and shut-in, and operations will continue tomorrow morning (11/9/17). No violations. 2011251853 2017-11-07 Plug / Plug Back No Violations Status check of plugging operations. Bishop Well Service has pulled the rods are beginning to lay down the 2.375 tubing to tally and inspect. I met with Doug Gonzales and discussed the plugging operations. It was decided to bring in another tubing string to set the bottom plug due to the oil and paraffin build up inside the string. The plan is to begin plugging tomorrow around 10:00am. No violations.
