1500985934 2017-07-25 Final Restoration No Violations Follow-up to Chiefs Order #2017-249 for failure to maintain liability insurance. I have been working with Jeff Moore (Moore Well Services) to determine that this well was plugged and abandoned. A plugging permit was issued to Kathy Butler (White Energy) on 6/3/1998 and expired on 6/3/1999. Jeff remembers plugging this well for Kathy Butler (White Energy) and has found some invoices for their service rig showing that they were on location for the job. The invoice clearly indicates that the well was plugged and abandoned on 6/19/1998 and was paid for by the developer, Bob Durst, of River Bend Inc. The invoice also indicates that Omega Cementing was the cementing contractor for the plug job. A Division Facility Report dated 9/13/1999 documents an inspection conducted by Inspector Johnny Patrick and states that he found 8.625 wellhead attached to approximately 10 feet of 8.625 casing laying on the ground south of the road. No evidence of well location. Division not informed that the well was plugged. My inspection today reveals that the area is now the River Bend Subdivision and is developed around the former location of the wellhead. I searched the area as best as possible and found no evidence that the well still exists. There is no lease road or tank battery in the area. Based on the collected evidence, this well was plugged and abandoned and any remnants of the former location have been overprinted by the development. Final Restoration is approved.
