0078420038 | 2010-05-05 | Salt Water Injection Well | No Violations | Met on site with Mark Smith and Wade Pol to discuss construction plans for this new facility. I advised them to follow the plan as submitted to Columbus and that any changes would require a written modification. Construction will begin as soon as the weather permits. |
0166432834 | 2016-06-21 | Salt Water Injection Well | No Violations | I inspected this salt water disposal well to see if the requested gauges had been installed at the wellhead. My inspection today found that no gauges have been installed. I inspected the pump house and surface facility to the south. The facility was currently open and PEP Drilling (#25303), Duck Creek Energy (#25331), and J Hawk (#25342) were delivering loads of salt water to the facility for injection. All trucks were marked and identified as required. I inspected the pump, which was currently running, and a gauge on the injection line registered an injection pressure of 680psi. I spoke with the facility operator and he will install the required gauges or repair the chart box at the wellhead. I will notify UIC Inspector Ochs of my findings. |
0178863005 | 2015-07-10 | Salt Water Injection Well | No Violations | The well and surface facility were inspected. There were no violations found at the time of inspection. |
0208556803 | 2010-06-22 | Salt Water Injection Well | No Violations | The concrete loading pad has been poured and the storage tanks are ready to plumb together. Work is also continuing inside of the building. |
0230308464 | 2010-06-02 | Salt Water Injection Well | No Violations | Pol has completed the installation of the 30 mil liner and the sand covering. The next step will cover the sand with 4 inches of crushed limestone (57s). The injection line has been installed but still needs the mechanical integrity test completed before use. Work is continuing on the building. |

0904553326 | 2010-04-02 | Salt Water Injection Well | No Violations | Appalachian Well Surveys set the cement retainer at 4440 on 4/1. Today, Petroset was on site to set the bottom plug by stinging into the retainer. However, as soon as the stinger entered the retainer, the injection pressure increased to between 1800 and 2000 psi. Repeated attempts failed to permit normal operations. The decision was made to pull the tubing and check the condition of the stinger. If the stinger is in good working order then the retainer will need to be removed. |

1117790914 | 2010-04-02 | Plug / Plug Back | No Violations | Bob Worstall called and wanted me to help him with the plug back. He had another meeting. The workover rig, Chipco had run in the hole with 2 3/8 tubing with stinger. Petro-Set pumped 54 barrels of fresh water to brake circulation. The rig crew dropped the stinger into the cement retainer. The pressure while circulated hole was 700psi. The pressure rose to 2000psi as soon the stinger went into the cement retainer. The stinger was pulled out of retainer. This process was preformed several times with the same result. Something is wrong with the cement retainer or the stinger is to short. Bob Worstall came on the location. PEP decided to pull out of the hole and try to drill up the cement retainer. |

1214704925 | 2016-05-12 | Salt Water Injection Well | No Violations | I conducted a quarterly status check inspection of this active injection well and the associated surface facilities with Inspector John Fleming. My inspection found the needle valves on the injection tubing and annular port between the 4.5 casing and tubing corroded and not fitted with gauges. I called Mark Smith (PEP) and asked him to place new needle valves and gauges on the wellhead so I could safely check injection and annular pressures. He told me he would be up on Tuesday (5/17/16) and install the valves and gauges. I will re-inspect the wellhead and facility after that time. |

1251658922 | 2017-10-31 | Salt Water Injection Well | No Violations | I found the UIC facility in good repair on the day of my inspection. The identification sign at the well is lying on the ground and the one at the facility was behind a wall and not in a conspicuous place. I spoke with the facility operator to repost the identification signs at the well and facility. There is a locked chain link fence around the well and facility. There are no violations at this time. |
1270124059 | 2010-03-31 | Production Wells | No Violations | I witnessed the setting of the cement retainer. The workover rig is Chipco LLC Well Service from Zanesville Ohio. I met with Mark Smith of PEP on the location. The logging company was Appalachian Well Service. They set the Owens 3.7 cement retainer at 4440. That is Two hundred feet above the top Clinton perf. There was a collar at 4450. The 2 3/8 tubing had parafin inside. Mark stated they would clean the parafin out of the tubing before going forward with the cement job. The cement job will be on Friday, 4/2/10. There were no violations at this time. |
1271076124 | 2010-04-09 | Salt Water Injection Well | No Violations | I witnessed the pressure testing of the casing. The permit wanted 1190psi for 15 minutes. Petroset pumped the casing to 1190psi. Lost 33psi in 15 minutes. There was some leakage around the 4 1/2 swedge to the casing. It could loose 59.5psi in 15 minutes. The pressure test passes. Rigged up Appalachian Well Service. Appalachian Well Service ran a GR/CCL/ bond log from 4430 to 3480. The top of the cement was at 3940. Called Tom Tomastik at Columbus and left him a message on the top of the cement. Will wait till Monday for the results from Tom. |
1271103556 | 2010-04-05 | Plug / Plug Back | No Violations | CHIPCO was on site to knock out the cement retainer that was placed in the well last Thursday to set the Clinton plug. Multiple attempts to set the stinger last Friday failed and the decision was made to removed the retainer and try again. |
1271186442 | 2010-04-06 | Plug / Plug Back | No Violations | Appalachian Well Service was on site to set a second cement retainer at 4424 for the purpose of squeezing the Clinton plug through six perforations from 4665 through 4745. CHIPCO ran 2-3/8 tubing in with a stinger to activate the tool. The system worked as designed and Petroset pumped 150 sacks of Class A cement through the retainer and into the formation. Pressure slowly built from 0 psi to 1200 psi during the mix and displace phases and reached a maximum of 1500 psi at plug down. The stinger was removed from the retainer and circulated clean before pulling the tubing string.
The next step is to complete a pressure test on the 4-1/2 casing and on the bottom plug and to run the cement bond log, all on Friday 4/9. The Newburg injection zone still needs to be perforated and fraced before the well can be used. |

1272901050 | 2010-04-23 | Salt Water Injection Well | No Violations | Successfully completed the required mechanical integrity test (MIT) on the tubing packer for this brine injection well. CHIPCO was on site to run a new Halliburton R-4 packer on a new string of 2-3/8 upset tubing. The well was perforated with 225 shots from 4136 to 4220. The packer was set in tension at 4120. Prior to setting the packer, Producers circulated the freshwater/inhibitor mixture down the annulus and up the tubing. The inhibitor mixture circulated to surface. Several attempts to complete the MIT failed, presumably due to air in the system. On the successful attempt the initial shut in pressure was 990 psi. After five minutes the pressure fell to 980 psi. After ten minutes the pressure fell to 975 psi. After 15 minutes, the pressure held at 975 psi. The packer and injection system PASSED the test. |
1275661355 | 2010-06-01 | Salt Water Injection Well | No Violations | W. Pol Contracting has completed the containment dike and base using clay under four inches of sand. They are starting to lay the 30 mil re-enforced liner which will be covered by another 4 inches of sand. The liner extends completely over the containment dike. The sand is being applied by a sling truck. The clay and sand were obtained from Charleston Sand and Gravel. |
1277155521 | 2010-06-03 | Salt Water Injection Well | No Violations | Pol is using a rock thrower to spread 57 limestone throughout the containment area. The tank stands with bankrun will be set next. |
