0162179627 2013-01-07 Completion Testing No Violations I was on location to check the status of the completion testing of this horizontal well. I met with Matthew Dye (CNX) to inspect the gas flaring operations. Pettigrew Pumping Service was handling the testing/producing of the well. Liquids (oil, brine, & flowback water) were being produced to the tank battery and methane was being flared through a stack at the east side of the location. A second stack was set up to the east to flare any vapors or gas produced from the tank battery. I spoke with Matt about the proper ID signage that is required and told him I would e-mail him all the pertinent information for urbanized production facilities. I inquired about the reclamation of the lower pad that was built for the quick-pits and I was told that the stone would be removed and the area reclaimed as soon as weather permitted. No violations.
0453085188 2017-02-17 UrbanProduction Wells No Violations I was on location with Inspector John Fleming to follow-up on a small gas leak found on the Baird valve between the tubing and casing flow lines. The valve appears to have been replaced and there was no evident gas leak at the valve. The tubing flow line valve at the wellhead remains locked out and closed. Active production was observed at the gas production unit and the inlet gauge registered 100psi. I found no violations at this time.

Calfrac and Oil States were on location rigging up the equipment. All the blenders, sand, and pump trucks were spotted on containment as well as the quick pits holding the freshwater. No violations.
0543310134 2012-06-29 Urban Drill / Deepen / Reopen No Violations I was on location and going over the last few days of operations with Sam Hindman (CNX). They drilled out from under 13.375 casing with 12.25 tri-cone bit to 525 KB (kelly bushing) on 06/27/2012. They discovered an issue with the annular BOP (blow-out-preventer) and brought in a new one. The annular was installed and passed the pressure test on 06/28/2012 and I received a copy of the test report. They are currently waiting on parts for the rotating head. New P-110, Range 3, 40 lb/ft, 9.625 casing was on site and inspected. No violations.

I picked up a copy of the BOP (blow-out-preventor) test performed over the weekend by Double Check Services, Inc. and reviewed it with Dan. All tests passed. Nabors crew was rigging up the 12.5 tricone bit to drill out the cement plug and 20 feet of hole prior to switching to the air hammer bit.
0697037086 2018-07-12 UrbanProduction Wells No Violations Status check inspection of this horizontal, Utica Shale well. My inspection found the access road gate locked and secure. The wellhead is fenced and equipped with 2.875 tubing and rods and operating with the aid of a Lufkin 640, electric powered pumping unit. Gauges indicate 255psi on the tubing and 325psi on the 5.5 production casing. Annular gauges indicate 0psi on the 13.375 surface casing and 110psi on the 9.625 intermediate casing and is not over-pressurized. I removed a steel plate three (3) feet north of the wellhead and found the 16 mouse/rat hole used during drilling operations. I ran a reel tape and weight into the hole and found that it was 60 deep and was not plugged as required following drilling operations conducted by CNX Gas Company. Northwood Energy Corp. became the owner of the well on 11/15/2017 and is now the responsible party to plug the open mouse/rat hole. I spoke with Northeast Regional Supervisor Tom Hill and he instructed me to give Northwood one (1) week to plug the hole. I spoke with Cory McCudden (Northwood) about the open mouse hole and he agreed that they would plug the hole, but may need more than a week to get a plan together for plugging the hole. I told him that as long as the mouse hole gets plugged in a timely manner, no violation would be issued. I will follow-up with Northwood next week.

Kelly bushing is 23 feet above ground level. Drlling rate of 1.5-2.8 feet per hour. I inspected the flare stack with Anthony Miller (head drilling engineer) that is being constructed for air operations. Unit consists of a vertical seperator with a liquid dump leading to a 50 barrel poly tank and a flow line leading to the flare stack that will be lit during air operations. The unit is more than sufficient to flare any sustainable gas shows encountered during drilling. No violations.
0979307517 2012-05-19 Drill / Deepen / Reopen No Violations I was on site today to witness the cementing of the conductor casing. Hole number 1 was 80 feet in depth and 42 inches in diameter. Casing was set at 80 feet. Casing was 30 inch. The hole was drilled in sand and gravel. The operator was having difficulty stabilizing the hole while drilling. Operator had drilled to 60 feet, the hole was caving. Operator stabilized the hole by filling it with cement. The operator used 80 yards of cement to stabilize the 60 foot hole. The operator then drilled through the cement and reached a depth of 80 feet. The operator did not reach bed rock. The operator was advised that not setting the conductor in bed rock could complicate drilling. The operator advised that they wanted to set the 80 feet of conductor to stabilize what hold they had made. Operator proposed to grout the conductor casing in place. I advised the company that grouting is only used as a remedial action. Due to the caving of the hole, the operator was instructed to wield a cement head onto the 30 inch casing, break circulation, condition the hole and circulate cement to surface. The operator was instructed to do this to insure a correct casing shoe. O-Tex cementing was on site to complete cementing. O-tex pumped 5 barrels of fresh water ahead of 15 barrels of gel and Baraseal. An additional 24 barrels of fresh water was then ran, circulation was established. 139 Barrels of class A cement/ 660 sacks was ran. 11 Barrels of cement returned to surface and held at surface.

1053060765 2012-06-17 Urban Drill / Deepen / Reopen No Violations I arrived on location at 7:20 A.M. for the 13.375 surface casing cement job. Nabors Rig M60 was still pulling drill pipe out of the hole. I left location and returned at 12:30 P.M. The casing crew began rigging up at 2:00 P.M. and started running surface casing at 4:00 P.M. I left location at 5:30 P.M. and returned at 9:30 P.M. Baker-Hughes cementers were rigging up for the cement job. Cementers began pumping at 12:30 A.M. and established circulation. Cement was pumped and good returns were achieved at the surface. The cement was plugged down at 1:15 A.M., held at surface, and shut in. See cement ticket for further details on the cement job.
1145914843 2019-02-04 UrbanProduction Wells No Violations Status check inspection of this horizontal, Point Pleasant well. My inspection found the access road stable and the lease road gate locked and secured. The location is stable and the snow has been plowed. The wellhead is fenced and equipped with 2.875 tubing and rods and an electric powered Lufkin 640 pumping unit. Annular gauges are present and there are no over-pressurized annuli. This well produces through a heated gas production unit to the south and to a concrete diked tank battery to the east containing six (6) 400 barrel tanks. Identification is posted at the pad entrance and at inconspicuous locations around the pad. I found no violations at this time.
1193426223 2013-09-05 UrbanPreliminary Restoration No Violations I was on location to check the status of preliminary restoration on this horizontal well pad. Drilling and completion equipment was removed from location in the fall of 2012. The lower pad used for equipment storage and a quick pit has been reclaimed. The limestone base was removed in the spring and the topsoil was respread, graded, and seeded. Vegitation is well established here and around the perimeter of the pad. Preliminary Restoration is approved. No violations.

1233459149 2012-10-05 Urban Drill / Deepen / Reopen No Violations I was on location at the request of James Black (Manager of Gas Field Compliance) to discuss the possibilties of discharging the freshwater in the quick-pit out into the field behind the location. All the water in the pit was purchased from a fire hydrant hook-up from Rootstown and, also, pumped from a small pond located behind Cracker Barrel. I had previously discussed this with Deputy Chief, Bob Worstall, and he approved of this as long as it does not cause any type of erosion. James and I determined that the best way to do this was to line a pre-existing, on-site ditch with limestone rip-rap and build a catch basin at the outlet that water could seep through. They would also attach a bag or diffuser on the end of the discharge line. Also, water samples would need to be taken and tested prior to discharge. I spoke with James later in the week to check on the progress of this and he informed me that CNX had decided that they would need the water at another location and would be hauling it by truck instead of discharging it. No violations.

1339697021 2012-06-13 Urban Drill / Deepen / Reopen No Violations On site for conductor casing #2 cementing, also on site to observe and assist DNR Inspector Mike Dinsmore. Hole #2 was 26 inches in diameter and the total depth was 260 Kelly Bushing (KB). Bedrock was encountered at 127 feet ground level (GL.) Operator set 194 GL of new, R-3, 20 inch, 94lb/ft casing with shoe and centralizers. Operator had a cement head on location with a wiper plug. Operator requested that they be allowed to cement and displace with a swedge, due to height issue with the cement head. Cementing and displacing through a swedge was allowed. Baker Hughes broke circulation, mixed cement and displaced. There was 11 barrels of cement returned to surface. Site condition: Sound barriers are up at the entrance of the site. Operator has constructed an above ground temporary water reservoir with two cells at the north end of the location. Approximate capacity of the two cells 50,000 barrels. 2nd temporary reservoir was under construction north west of the rig. Site was stable. Drainage controls were in place. Permit was in the superintendent office on location. Security was on location at the entrance.

Nabors Rig M60 ran 114 joints of new, P-110, intermediate casing with sixty centralizers to a depth of 4926 at the kelly bushing. Baker-Hughes was rigged up and ready to cement. A shift change on the Nabors Rig required an additional safety meeting and CNX realized that they needed to haul in additional freshwater for the cement job. I was unable to witness the remainder of the cement job.

1355414494 2012-12-13 UrbanProduction Wells No Violations I was on location with inspector Bob Roush and Matthew Dye (CNX) to inspect the progress of the installation of the production equipment on this horizontal well. A crane had spotted a Lufkin 640 pumping unit on the east side of the wellhead and was preparing to install the horse head on the unit. A cement dike had been constructed for the tank battery and had 4 concrete wall and a concrete floor. Six 400 barrel tanks had been spotted inside the tank battery and each were placed on gravel filled steel catchment. Spacing between the tanks was 3+ feet and satifies set back rerquirements. The production unit (GPU) had not bee spotted and a cement, diked pad had been poured for the unit. There were sumps in the tank and the GPU pad, and I let Matthew know that they would need bull plugs or ball valves on the ends to keep them closed. Spacing between the wellhead, tanks and GPU was more than adequate. Matthew told me the pumping unit will be operated by an electric, propane generator until a 3-phase electric drop can be obtained and should not be a noise issue. The security shack has been removed and a gate has been placed at the entrance to the well pad. The gate will be locked when no one is on location. They are currently having locksets made and I will be given a key for their locks when they are ready. All drilling equipment has been removed from the location. There is a portable pumping unit and flare stack still present on location. They have not tied into a gas line as of today. Matthew told me they should be finished setting up equipment in 3-4 weeks. No violations.
1364224570 2013-03-20 Urban Drill / Deepen / Reopen No Violations Inspector Mike Dinsmore asked me to check out a gas odor complaint. I arrived at the above location and Pettigrew Pumping was on location. I told them that I was responding to gas odor complaint in the area. I could not detect or smell any gas odor at this location. The wind was blowing to the southeast.
1434030743 2015-04-29 Preliminary Restoration No Violations I approved urban preliminary restoration on 9/5/2013 under inspection #1193426223, but it has not dropped off the activities list. I will conduct this inspection as preliminary and not urban restoration. The wellhead is equipped with a Lufkin 640 electric pumping unit. The area around the wellhead and pumping unit is fenced with appropriate warning signage posted. The stuffing box is enclosed with a plastic spill catcher. The tank battery is surrounded by a 5 high, concrete dike and contains six (6) 400 barrel tanks. The tanks are plumbed together and are equipped with charcoal filters and lightning arrestors. The load lines are equipped with pumpkin spill preventers and are locked. The top of the tanks are vented through an automatic vapor recovery unit to the east which is fenced. The drainage sump in the southwest corner of the tank battery is closed. The gas production unit is surrounded by concrete containment barrier and the sump is closed and equipped with a bull plug. There are multiple emergency shut down stations on location. The pad is fenced with post and wire and the access gate is locked. Well identification is posted at the entrance to the pad. All drilling and completion equipment were removed in the fall of 2012. The lower pad to the northwest that was used for quick pits and equipment storage has been completely removed. The stockpiled topsoil was spread, graded, and seeded. Native grasses are well established in the area as well as those areas surrounding the pad. Preliminary Restoration is approved. No violations.
1481569004 2012-07-09 Urban Drill / Deepen / Reopen No Violations On location with DNR Deputy Chief Bob Worstall to review the bond log pertaining to the intermediate (9.625) casing cement job from 7/7/12. We met with Anthony Miller (CNX) and reviewed the log. It was determined that the top of the cement was at 1300. No remedial action was required, and we were satisfied with the results of the cement job. We inspected the location and examined the water impoundments that had been constructed. All air drilling equipment is being removed from the location at this time. No violations.
1487260446 2017-02-10 Production Wells No Violations I performed a follow up inspection on this location. Inspector Mike Dinsmore discovered a leak around the Baird Value on the tubing flow line. He called CNX personal about the leak. The said that there field personal would be on location to repair the valve on 2/9/201.Mike was out of town the following day and ask if I would do the follow up. Up on my inspection I could not find the leak at that time. The repairs were made. The well is equipped with a pumping and an electric motor. The tank battery is equipped with six 200 barrel tanks and one GPU.
1488480523 2017-02-08 UrbanProduction Wells No Violations I was on location to check the status of this horizontal, Utica Shale well. My inspection found the lease road and well pad in stable condition and the access gate locked. This well is equipped with a Lufkin 640, electric-powered pumping unit. The tubing flow line valve at the wellhead is currently closed and has been locked out and tagged out. The casing flow line valve is open and is producing gas. I found the screw valve (Baird) between the tubing and casing flow lines frosted over and leaking a very minor amount of gas. The leak does not currently pose a threat to public health or safety and I notified Pete Nichol (CNX) of the leak. He told me that someone would be out this evening or first thing in the morning to repair the leak. The remainder of the well pad consisted of a diked tank battery containing six (6) 400 barrel tanks, a small vapor recovery unit, and heated gas production unit. I will check back later on the leak repair.
1501503266 2017-07-31 UrbanProduction Wells No Violations I conducted a status check inspection oh this horizontal, Utica Shale well. My inspection found the well equipped with an electric powered pumping unit. The tubing valve is closed and locked out and the casing valve is open and in production. The tank battery is diked, the sump is closed, and there is 3 of water in the northwest corner of the battery over the sump. The battery contains six (6) 400 barrel tanks and are equipped with lightening arrestors and grounded. The tank vents are plumbed to a vapor recovery unit to the south and the unit is not currently operating. The GPU is located on the west side of the pad and there is natural gas moving through the unit. The pad entrance is locked and the required identification is posted at the tank battery and the access road. I found no violations at this time.
1523967303 2018-04-16 UrbanProduction Wells No Violations Status check inspection of this horizontal Point Pleasant well. My inspection found the well pumper, Chris Scofield, on location and producing the well. He told me he has run the pumping unit several times, but is limited due to the lack of capacity at the sales point. I spoke to him about keeping the access gate at lease road entrance closed and locked when not on location. He agreed and gave me the combination to the new lock. The sign at the lease road entrance has been updated to reflect Northwood Energy Corp. as the owner of the well and the required emergency number has been posted. No violations.

My inspection found the hole has been plugged as requested (I was not notified). There is cement covering the ground in a 20 x 20 area around the wellhead, annular valves, and across the cellar. Hard cement was tagged five (5) inches below the top of the 16 casing used to line the hole. I spoke with Field Supervisor Chris Schofield and he told me that Don Caporaletti brought in a grout truck on Wednesday (7/25/18) and filled the hole. There are no violations at this time.
1796868759 2012-06-13 Urban Drill / Deepen / Reopen No Violations On location with DNR Inspector Tom Hill to witness cementing of the 20 conductor pipe. The 26 hole was drilled to 260 KB (Kelly Bushing). Once the power tongs arrived, the casing crew ran 6 joints of new 20, J-55, 94 lb/ft conductor pipe. The cementers then rigged up, broke circulation and cemented the conductor pipe. They achieved 12 barrels of good returns. See cement ticket for further details. No violations.
1863897777 2012-09-17 Urban Drill / Deepen / Reopen No Violations I was on location to inspect and check the progress of the hydraulic fracturing of the Utica/Point Pleasant interval in this horizontal well. I met with Jeramiah Alton and Steve Spitler (CNX) in Calfracs Comm center trailer. They are currently treating stage 8 of 16 zones to be completed in this well. Treatment pressure is averaging around 8400psi and the annulus is being continuously monitored for communication. There have been no occurrences of annular pressure. This stage was treating tight and they were unable to put away any 100 mesh sand so they switched to 20/40 mesh to treat this stage. All equipment is on containment and the location is in good condition. No violations.
1965782600 2012-07-11 Urban Drill / Deepen / Reopen No Violations I was on location to inspect operational progress on the well. I met with Wes Martincic (CNX) and was informed that they drilled the hole to 5763. They are currently tripping the drill string out of the hole and assembling the BHA (bottom hole assembly) that will be used for coring the next 390 section for analysis. No violations.
