0100479666 2014-12-16 Casing String Witnessed No Violations I was on location to witness the 11.75 conductor casing cement job. Poulson Drilling drilled a 14.75 hole to a depth of 75 and ran 58 of conductor casing into the hole. Petroset rigged up and cemented the casing with 11.6 barrels of Class A cement slurry. 0.5 barrels of cement circulated to the cellar and the cement head was shut-in. 0100705647 2015-06-26 Preliminary Restoration No Violations I inspected this well for preliminary restoration which was due by the second week of June 2015. The well is equipped with an electric pumping unit and is currently operating. Gauges indicate 350psi on the 4.5 casing and 175psi on the 2.375 tubing. The proper ID is posted at the wellhead. All drilling equipment has been removed and the pits have been solidified and buried. The stockpiled topsoil that was located on the west and north sides of location has been spread out and the location has been graded and seeded. I inspected the tank battery and the diked containment area is shared by the Lee #s 2 & 3 (PN#s 1895 & 1896). This well produces to a horizontal separator and into one (1) 140 barrel tank, one (1)-100 barrel tank, and one (1)-70 barrel tank. The proper ID is posted at the tank battery. Preliminary restoration is approved. No violations.
0243393969 2014-12-29 Drill / Deepen / Reopen No Violations I was on location to check the drilling status of Poulson Rig #1. They have drilled the 7.875 hole to a total depth of 4610 and are currently tripping drill pipe out of the hole. I inspected the used, 4.5 production casing on the trailers, obtained the drift and pressure testing paperwork from the operator, and it was passed. They should be cementing casing this evening. The pits are in good condition and have plenty of freeboard. No violations.
0550592386 2015-04-13 Preliminary Restoration No Violations I inspected this well for preliminary restoration which is due by the second week of June 2015. The well is equipped with an electric pumping unit and is currently operating. Gauges indicate 500psi on the 4.5 casing and 100psi on the 2.375 tubing. The proper ID is posted at the wellhead. All drilling equipment has been removed and the pits have been solidified and buried. There is one (1) stick of flow line, three (3) joints of tubing, and four (4) pumping rods on the south side of the location that need removed. Silt fencing remains in place along the south and east sides of the location. The stockpiled topsoil on the west and north sides of location need to be spread out, graded, and seeded. I inspected the tank battery and the diked containment area is shared by the Lee #s 2 & 3 (PN#s 1895 & 1896). This well produces to a horizontal separator and into 1-140 barrel tank, 1-100 barrel tank, and 1-70 barrel tank. The proper ID is posted at the tank battery. Preliminary restoration is not passed at this time. No violations.
0999150921 2014-12-17 Casing String Witnessed No Violations I was on location to witness the cementing of the 8.625 surface casing. Poulson Drilling drilled an 11 hole to a depth of 430 and ran 407 of surface casing into the hole. Petroset rigged up, broke circulation, pumped gel and cottonseed hulls, and then mixed and pumped 29 barrels of cement slurry. They dropped the wiper plug, and displaced the plug to 387 with 24.5 barrels of water. 11 barrels of cement circulated to surface. See casing ticket for more details on this job. No violations.

1168851847 2015-04-01 Preliminary Restoration No Violations I was on location to check the preliminary restoration status of this Clinton well that was completed in January 2015. Due to the long winter, restoration operations have been delayed. The drilling pits were solidified and closed in January. Northeast Oilfield Services is currently hauling out 3-500 barrel frac tanks. I spoke with the contractor, Logan, and he informed me that the well was just turned on. I did not inspect the well at this time due to trucking activity. Logan said he will be working on reclaiming the site as the weather allows. Preliminary restoration is due in June. No violations.
1425490059 2014-12-19 Drill / Deepen / Reopen No Violations I was on location to check the drilling status of Poulson Drilling Rig #1. Roy and Larry Poulson were shutting the equipment down and preparing to lock things up until after the holiday. The rig shut down a depth of 2465 before reaching the top of the Big Lime. They will resume drilling operations on 12/27/14. All the equipment has been secured and the drilling pits have plenty of freeboard. No violations.
