Well identification is missing and according to 1501:9-9-05(A)10/1509:23 the well identification sign shall include owners name, county, township, well name and number, permit number for the well, and emergency contact number. There is no tank battery associated with this well. Refer to inspectors report # 1361547260 on the leaking condition of the well and required compliance date for remediation of this well.
0554163906 2018-02-12 Production Wells No Violations On the day of this Inspections, I found a gas well consisting of 5.5 inch diameter casing, and 2 inch diameter tubing. Top of the wellhead has tubing connected to drip laying above ground and is connected to a 1 inch diameter orange flowline. The well has been brought in to compliance for failure to legibly identify well and is now in compliance.

The tubing is rotten off at the tubing head. Well not leaking from the surface casing head. Bubbles and fluid seen inside the tubing. I attempted to speak with the landowner but no one was at the residence.

1361547260 2013-02-06 Production Wells Violations Noted Theres production tubing rising out of surface casing. There is no plug on the end of the production casing. Gas is bubbling through water in the tubing and venting out the end into the air. The PhD6 gas meter detected .9 meter reading units for CH4 (methane) and 6 meter reading units for CO coming out of the tubing. According to 1501:9-9-05(A)1 /1509:20 the well is insufficiently equipped to prevent venting gas into the atmosphere and release of brine. Checked water ponded around the surface casing for chlorides with the 30- 600 ppm Quantab. Chlorides were detected above 250 ppm. Well shall be repaired to prevent pollution of the environment. There is no tank battery associated with this well. Wellhead located in a corn field and can easily be seen from both roads that define the north and west borders of the field.
