Poulson Drilling drilled a 22 bore hole to 63 GL on a fresh water fluid system. Poulson Drilling ran two joints of 16, 54lb/ft, short thread coupling casing to 55 with one bow spring centralizer. Petroset Cementing Services (PCS) connected to the casing and established circulation, conditioned the bore hole with 200 lbs. of gel and 40 lbs. of cottonseed hulls. PCS mixed a cement slurry consisting of 230 sacks of Class A cement blended with 3% CaCl. Cement slurry verified by scale at 14.7 ppg. The cement slurry was displaced to 45 GL with 10 barrels of fresh water. I observed approximately 12 barrels of cement slurry returned to surface. No violations during this inspection. |

Canton Oilwell Services were clearing the location of the trees and brush. Joe Little Excavating was putting in the filter socks and silt fencing. I sampled the creek water from which the cement mix water was to be pumped from and found it to have a pH of 4. The creek meandered through a peat bog and leached the excessive acidity from it. I spoke with Rob Frock ( East Canton Energy ) and informed him that this water was outside of parameters for mix water. Mr. Frock agreed and said would haul the water in. No violations identified during this inspection. |

Poulson Drilling drilled a 14.75 bore hole from 64 to 428 while circulating a fresh water based drilling fluid. The Big Injun Sandstone was found from 250 to 350 GL. Poulson Drilling placed 10 joints of 11.75, 44 lb/ft, short thread coupling casing and two bow spring centralizers to a depth of 403 GL. Petroset Cementing Services (PCS) connected to the casing and established circulation. PCS mixed a cement slurry consisting of 300 sacks of Class A blended with 3% CaCl and 40 lbs of cottonseed hulls and circulated it back to surface. Cement slurry verified by scale at 15.0 ppg. The cement slurry was displaced to 383 GL with 47 barrels of fresh water. I observed approximately 21 barrels of cement slurry back to surface. No violations during this inspection. |

The location was built on a glacial feature comprised of sandy soil. Joe Little Excavating was the contractor for the dirt work. The pits were dug with the sides having an angle of repose approaching 60 degrees in order to stabilize the sidewalls. The pits were double lined with pit liners. An excavation was made at the well stake down approximately five (5) feet deep and six (6) feet X six (6) feet square hole. A pit liner was positioned in the hole and a four (4) feet X five (5) feet plastic culvert was placed on end and on top of the liner. Ten (10) sacks of Sakrete cement was mixed and dumped around the culvert to seal the cellar pipe. The pit liner was long enough to reach the working pit. Thus allowing a liquid tight circulation path. I did not identify any violations during this inspection. |

PCS mixed a cement slurry consisting of 200 sacks of Class A cement blended with 2% calcium chloride. The cement slurry was displaced to 4506 GL with 72 barrels of fresh water. 1717691293 | 2016-10-31 | UrbanProduction Wells | No Violations | I inspected the Holy Spirit Catholic Church Unit # 1 to assess the restoration and construction activities. The well head and tank battery was accessed on a gravelled road. The well was configured to operated as an electric pumping well. The 8.625 annular monitoring gauge was absent and I was told by Rob Frock that the pumper has the gauge and will install it. The tank battery is located west of the well head and consists of one (1) 210 barrel oil tank, one (1) 100 barrel steel brine tank and a vertical separator. The tank battery was surrounded with an earthen dike. All equipment spacing rules were observed to meet the requirements. The 210 tank had the charcoal filter, lightening arrestor and pressure relief hatch in place. The fencing requirement has not been fulfilled at this time, but will be once the initial restoration is completed. The landscaping requirement was waived by the DOGRM Inspector. Joe Little Excavating was contouring the location with the top soil. I asked Mr. Frock to correct the silt fencing and maintain the same, in working condition, until the location has established vegetative growth and is stabilized. No violations identified during this inspection. |
