Moore Well Services (MWS) was the plugging contractor and Petroset Cementing (PC) was the cement contractor. MWS pulled the remainder of the 4.5 production casing from the well (66 joints total). The ran in with the knife tool and jars to 2420 and confirmed that the Big Lime plug had been pumped out of the hole and that there were no obstructions. The tubing trailer arrived and plugs were spotted across the Big Lime, Berea Sandstone/Ohio Shale interval, and across the glacial deposits. Appalachian Well Surveys set a cast iron bridge plug at a depth of 240 prior to setting the top plug across the glacial deposits. No violations. See plugging report for more details on this job. |
0690420420 | 2017-08-02 | Plug / Plug Back | No Violations | I was on location at 7:40am to witness plugging operations on this Clinton Sandstone well. Keith Moore called me and told me they were working on the power tongs and that it could be several hours before they were able to get to location. The previous day (8/1/17) a hole was suspected in the 4.5 production when cement circulated to surface when setting the Big Lime plug. I inspected the casing and found a 10 split on the 17th joint pulled from the hole. I measured the split to be 23 above the pin end of the casing including the threads. I also found two (2) small holes ten feet above the pin end. The split and the holes all occurred along the weld seam of the casing. I found that Moore Well Services (MWS) had dug an unlined pit and placed the circulated cement from the failed plug into the pit. There was currently 12-14 of water in the pit with crude oil and emulsion on top of the water. I asked MWS to bring in their water truck and suck the product out of the pit. The water truck arrived and the pit was emptied as much as possible and a bag of peat moss was spread across the bottom of the pit and sidewalls to lock up any free oil. MWS will need to excavate any contaminated solids and oil soaked material prior to closing the pit. I spoke with Keith Moore and the tongs will not be fixed for operations today. They will be on location at 8:30 tomorrow (8/3/17) to continue operations. |

The tank battery is a common battery with Walsh # 3 and Walsh # 4. The tank battery consisted of six (6) 100 barrel tanks, three (3) 50 barrel brine tanks, three (3) vertical separators and three (3) gas measurement stations. Moore Well Service was the contractor for the removal and restoration. The tank bottoms I inspected appeared to be free of holes and leakage. No violations were identified during my inspection. |
1158492200 | 2009-03-31 | Production Wells | No Violations | I received a call from Dominion East Ohio reporting a natural gas leak on the north side of the Walsh Jesuit High School property. The leak was determined to be associated with the Walsh wells operated by Moore Well Service. Moore responded with personnel and equipment to expose and clamp the leaking line. The last reported production from this well was in 1996. I spoke with Jeff Moore about any plans for this well. He said that he would review the history but in all likelyhood, would plug and abandon in the near future. |
1501874004 | 2017-08-01 | Plug / Plug Back | No Violations | I was observing the plugging of the Walsh 3 2 well when I noticed cement slurry returning to the steel pit at approximately 30 barrels into the displacement ( 32 barrels ). I notified Keith Moore ( Moore Well Service ) and Paul Hesse ( Petroset Cementing Services ) of my findings. Mr. Moore decided to evacuate the well bore of the cement slurry by pumping a volume of water exceeding the hole capacity. It appeared that the work string ( 4.5 casing ) had developed a breach, uphole, and the entire cement plug was circulated out during the displacement. After removing 17 joints of casing the power tongs broke and the operation ceased, after ensuring that the casing was free in the hole. Plan forward was to remove the remaining 4.5 casing and check the competency of the Big Lime plug. |

They dismantled the well head and pulled approximately 3776 of 1.5 tubing to removed the mud anchor. The tubing was put back in the 4.5 casing to function as a work string to spot the Clinton plug. Petroset Cementing Services ( PCS ) conditioned the well with 69 barrels of fresh water until circulation returns were clean. PCS then mixed and displaced a cement slurry comprised of 35 sacks of Class A cement ( mix water was 78 F and pH was 7 ) . Circulation was maintained throughout the job. MWS removed 30 joints of the 1.5 work string and shut in the well overnight. |
