1554739903 | 2019-03-27 | UrbanProduction Wells | No Violations | I received a phone call from Don Nething ( OVE, 330.283.5758 ) to alert me that a two (2) inch valve had failed and allowed the contents of the 140 barrel stock tank to be emptied into the diked area of the tank battery. Mr. Nething indicated that the Incident Notification number was called to alert Emergency Response.The closing gauge on the tank indicated that approximately 79 barrels of fluid was spilled. The majority of the spill was contained within the diked feature, but a minor amount of fluid was found outside of containment. This fluid had traced the path of the oil load line through the dike and covered an area of 10 X 60 towards the west. Ohio Valley Energy mobilized excavation equipment to dig an interceptor ditch perpendicular to the flow and construct a reverse syphon dam for containment in anticipation of heavy rains expected that night. A low range QuanTab indicated 550 ppm Chlorides 70 west of the tank battery. The site was stabilized and operations were shut down overnight. |

DOGRM was represented ( at various times ) by Rob Leidy ( Radiation Group ) Andrew Adgate and Fred Romeo ( Emergency Response ) and Bob Roush ( Summit County DOGRM Inspector ). The tank battery equipment was dismantled and steamed clean of all contamination. An area of bermed containment was constructed on which one (1) 140 barrel stock tank, one (1) 35 barrel poly brine tank, one (1) 2408 horizontal separator and ancillary equipment was placed. An excavator stockpiled the tank pad gravel in order to steam the rock prior to removal. A vacuum truck worked inside the tank battery dike to remove the free liquids caused by the steaming procedure. The activity continued until approximately 5:00 pm when the threat of rain shut down the operations. OVE personnel decided to shut down over the wekend because of weather concerns. Site was secured. |
