0002079987 2009-02-02 Drill / Deepen / Reopen No Violations Dons Excavating is on site building the lease road and grading the location. The rig is scheduled to arrive within the next few weeks. The required signage and the drilling permit have been posted.
0051371437 2009-04-28 Drill / Deepen / Reopen No Violations Dons Excavating is on site to grade the site. The tank battery has been built but the required fencing is not in place.
0128923744 2009-03-09 Drill / Deepen / Reopen No Violations The pits have been solidified and closed. The frac tanks are on site and the frac job is scheduled for tomorrow. No violations at this time.
0183739604 2009-02-27 Drill / Deepen / Reopen No Violations The drilling rig has moved off site. The pits are fenced and lined but need to be pumped down. According to Don Nething with Ohio Valley, the fluid level in the pits will be lowered today.
0355265129 2009-02-18 Drill / Deepen / Reopen No Violations Wildcat Drilling is moving on the location and will spud the well tomorrow morning. Both the conductor casing and the surface casing will be cemented to surface.
0551119010 2009-03-10 Drill / Deepen / Reopen No Violations At 10:36 am on 3/10, I received a call from Dale Nething with OVE reporting an unknown failure at the well site and requesting my immediate response. I arrived on site at approximately 11:15 and met with Rick Liddle and Tom Wood. Universal Well Service was on site to frac the well and J-Well was on site with the service rig. According to Rick Liddle, the well broke at 1200 psi. Universal balled off and then shut down for 30 minutes for the balls to fall off. Universal started in with the nitrogen foam pad and had mixed and displaced approximately 2/3 of the planned volume (250,000 scf). The pressure was 2230 psi when the well started circulating gas out the backside. Universal shut down immediately and J-Well plumbed both the 4-1/2 and the backside directly into a 300 bbl frac tank. According to Tom Wood, the 4-1/2 casing came from McJunkin and was classified as limited service, 11.6 ppf tested to 4900 psi. The limited service designation was due to mixed R-2 and R-3 lengths. Reportedly, at no time was the backside shut in. The frac crew was released and Universal sent a pickup truck with a wireline spool and a pump truck to pump a flag down hole to try an locate the problem. Universal pumped 31 bbl and displaced the flag to 426. The well never circulated fluid but was gasing (presumably nitrogen). The maximum pressure was 150 psi at a maximum rate of 4 bpm. The well was plumbed into a frac tank and left open over night. Baker Atlas will be on site tomorrow to run 1) cement bond log, 2) casing integrity log, 3) retrievable bridge plug and 4) a casing cutter in the first joint below the bad spot.
0554657247 2009-03-19 Drill / Deepen / Reopen No Violations The well was fraced on 3/18. Today, J-Well ran tubing and continued to flow back the well.
0584056619 2009-06-18 Drill / Deepen / Reopen No Violations The affected area has been graded and the fence posts are in place. The well can be turned on as soon as the fencing is installed.
0756256346 2009-02-19 Drill / Deepen / Reopen No Violations Wildcat is drilling the conductor hole. The suction pit has been dug and lined and Dons Excavating is digging the blow pit. Should be ready to run and cement conductor casing later today.
1236001351 2009-02-24 Drill / Deepen / Reopen No Violations Wildcat is drilling ahead at 3560. They should TD the well tomorrow and move on Thursday 2/26. The pits are fenced and lined as required.
1237824214 2009-03-17 Drill / Deepen / Reopen No Violations Frac job begun last week was completed today. The casing patch held and there was no evidence of any problem with the 4-1/2 casing.
1238770714 2009-03-31 Drill / Deepen / Reopen No Violations The tank battery location is being built and site restoration has started with rough grading and filling. The repair of the damaged asphalt parking lot is also being scheduled. No violations at this time.
1252080904 2009-02-11 Drill / Deepen / Reopen No Violations Dons Excavating is on site building the lease road and the drilling location. The rig is scheduled to arrive next week. The required identification and permit are posted at the entrance.
1524766959 2018-04-26 UrbanProduction Wells No Violations Access to the wellhead and tank battery was on paved and graveled surfaces. The well was operated as an electric pumping well. A locked board on board fence secured the well. I.D. signage was posted and legible. The wellhead had some lubricating oil on it from the polish rod lubricator. The 8.625 X 4.5 annular space was monitored in the tank battery. The tank battery was secured with a locked board on board wooden fence and an earthen dike. I.D. signage was posted and legible. The tank battery consisted of one 91) 140 barrel stock tank ( with carbon filter, lightning arrestor and pressure relief hatch ), one (1) 50 barrel poly brine tank and one (1) 2408 separator with gas meter. The load lines were locked and plugged. No leaks were identified. No violations found during this inspection.
1588865200 2009-05-11 Drill / Deepen / Reopen No Violations The wellhead and tank battery have been painted and the site has been graded. The well can be turned on as soon as the required fencing has been installed.
1732710277 2009-03-11 Drill / Deepen / Reopen No Violations Baker Atlas was on site to run a cement bond log and a casing integrity log. The bond log indicated that the top of cement is into the salt section. The integrity log indicated that there is a split in the casing from 424 to 426. Atlas set a retrievable bridge plug at 3525 and cut off the 4-1/2 at 462 which is in the first full joint below the split. The next step will involve setting a 4-1/2 overshot on the cut end and pressure testing the repair before rescheduling the frac job.
