0402912589 2012-01-17 Urban Drill / Deepen / Reopen No Violations FOLLOW UP TO NOV # 1326829506 The Church of New Hope well was drilled in 2011 and was permitted on 20 acre spacing. Upon reaching TD and logging the well, it was discovered that the Clinton sandstone was deeper than 4,000 and therefore the well would require a 40 acre drilling unit. PEP was given until the expiration date of the drilling permit (12/29/2011) to acquire sufficient additional leases to meet the acreage and spacing requirements for a 40 acre drilling unit. The following summarizes activity at this location:

1. The drilling permit was issued on 12/9/2009 and expired on 12/9/2010 2. The drilling permit was reissued on 12/30/2010 and expired on 12/29/2011 3. PEP Drilling spudded the well on 8/25/2011 4. Surface casing was set and cemented on 8/27/2011 5. PEP Drilling reached first TD at 4,037 on 9/1/2011 6. The open hole nuclear logs indicate that the base of the Packer Shell is at 3,988 7. PEP Drilling reached final TD at 4,161 on 9/3/2011 8. The open hole nuclear logs indicate that the base of the Clinton sand is 4,074

To date, there has been no official documentation provided to the Division with proof that Pursie Pipes has acquired the necessary leases to meet the minimum acreage and spacing requirements. The well must be plugged and abandoned and the affected area reclaimed.

0522772635 2011-11-15 Urban Drill / Deepen / Reopen No Violations No change in the surface condition at this location. The affected area has not been graded or seeded. PEP is still working to acquire the necessary acreage to make this a legal 40+ acre drilling unit. Columbus has given PEP until the end of December (drilling permit expiration date) to complete the process.
0908898688 2013-01-10 UrbanProduction Wells No Violations Met with Mark Smith ( PEP Drlg. ) to determine if any H2S gas is present in the annular space. The 8.625 casing valve was opened and a Gas Detector was placed in the gas stream. The Gas Detector did not alert to the presence on H2S gas in the annular space. A fencing crew was on location to build the required fencing around the wellhead, tank battery and Dom/EOG gas measurement station. Annular casing valve with a pop-off pressure relief was in place and functional. All activities on this location were found to be in compliance.
1277324348 2011-09-06 Urban Drill / Deepen / Reopen No Violations On 9/1/11, PEP logged the well and found that it was not drilled deep enough. The well was drilled on 20 acre spacing but the top of the Clinton was below 4000. After much discussion about the likelyhood of obtaining the additional acreage and footage required to produce a formation deeper than 4000, PEP ran drill pipe back in and drilled another 140 approximately. On 9/2/11, they logged the well again and ran 4.5 casing. They are not permitted to complete the well until the additional acreage is obtained and the drilling unit is revised. Preliminary Restoration is due by the first of December.
1315061575 2011-08-29 Urban Drill / Deepen / Reopen No Violations PEP Drilling set surface casing early on 8/27. After waiting on cement and completing the BOP test, PEP resumed drilling the main hole with fresh water. On this date, they were drilling at 2920. The pits are fenced, lined and in good condition.
