1116802606 2018-05-01 UrbanProduction Wells No Violations Access was gained on a asphalt and gravel driveway. The well was operated as a electric pumping well. The wellhead was surrounded with a chain link fence. The fence was locked. The well was not operating during this inspection. All valves were in the open position. A domestic manifold and meter was connected to the 4.5 casing. The tank battery was surrounded with an earthen dike and locked chain link fence. The required I D signage was posted and legible. The tank battery consisted of one (1) 210 barrel stock tank, one (1) 100 barrel brine tank, one (1) 2408 horizontal separator and one (1) meter station. Proper spacing between equipment was observed. The load lines were plugged and locked. No leaks were found. No violations.

1780373710 2010-07-13 Drill / Deepen / Reopen No Violations I received a call from Tim Levengood reporting that the cement inside of the 4-1/2 casing covered the interval to be perforated. One of three things may have happened. 1) the cement engineer did not displace with the necessary volume of water. The calculated volume was approximately 62.5 bbl. The operator reported displacing cement with 65 bbl of water. 2) the latchdown plug did not set as designed. The operator reported setting the plug with 450 psi. 3) a hole or split in the 4-1/2 casing diverted some of the displacement volume. Levengood requested permission to use the drilling pit to circulate into while drilling out the cement inside of the 4-1/2. Permission was granted.
