-open position. The diked tank battery has a 100 bbl. storage tank. The load line is secure with a bull plug. The separator is outside of the dike. There is a chain link fence around the well and production equipment. There are locks on the two (2) man-doors on the fence. Identification signs are posted on the 100 bbl. storage tank and fence with no well information. I spoke with Steel & Alloy LLC. and instructed them to post the proper well information on the two (2) identification signs. A termination letter for Chiefs Order 2016-410 was issued on February 6, 2017. A follow up inspection will be conducted for the identification signs by March 1, 2017.

0649063955 2014-05-19 UrbanProduction Wells No Violations I was on location for a follow-up inspection in reference to Chiefs Order 2014-66 and compliance notice #968348499. I found during my inspection chains and locks installed on the chain link gates. There was an identification sign posted on the production tank. I called and spoke with Mr. Gallo concerning bonding and insurance. Mr. Gallo stated that an owners number and bonding and insurance had been obtained. Mr. Gallo stated that Jay Adams of J-Well just received the Form 7 paperwork from ODNR which would be signed and completed by USX Realty and Steel & Alloy Utility Products then returned to ODNR. I will continue to follow-up on the Chiefs Order.
0968348499 2013-10-24 UrbanProduction Wells Violations Noted My inspection found the well not in production. The well was equipped with a plunger lift unit. The tank battery contained a 210 barrel production tank and a heat, treated separator. All production equipment spacing was incompliance. There was a chain link fence surrounding all of the production equipment and well with two gates and no locks. There was no identification sign posted. The lease road was part of the factories storage yard. The well was placed into production prior to House Bill 278. Our database indicated USX Realty has failed to file annual production reports. The last annual production was reported in 1984. I called USX Realty Development at 412-433-7996 and left a message that a compliance notice would be issued to produce, file for a plugging permit, or file for temporary inactive status by 12/13/2013. Post the proper identification sign by 11/15/2013. I have attached an annual statement of production (Form 10), please complete and submit by 12/6/2013.
1188588563 2014-06-04 Administrative Inspection No Violations I received a call from Jay Adams of J-Well Service in reference to Chiefs Order 2014-66. Jay stated he delivered the form 7 to Steel & Alloy Utility Products. Jay said the company would immediately sign the form and mail the paperwork to ODNR.

First, I called USX Realty and left a message for a return call in relation to the Chiefs Order. Then, I called Jay Adams of J-Well concerning the Chiefs Order, who was maintaining permit number 2-0432 for USX Realty. Jay stated USX had received the Chiefs Order; however, Jay claimed that USX Realty had sold the property where the well was located to a business called Steel & Alloy Utility Products. Next, I received a call from a Nick Gallo of Steel & Alloy. Nick stated Steel & Alloy received the Chiefs Order from USX. Also, Nick stated Steel & Alloy was working with Jay Adams with filing the correct paperwork for an owner number and insurance and bonding. Steel & Alloy plan to file for non-domestic, since the well was being used to heat their business. Nick stated the paperwork would be filed immediately. Next, I told Nick that there was a current compliance notice on file. I instructed Nick to post the proper identification sign and place locks on the fence gates. Lastly, I stated Steel & Alloy had 30 days to comply with the Chiefs Order on all violations.
