0039268587 2016-06-01 UrbanProduction Wells No Violations I conducted a civil action follow-up inspection on this well. The well was equipped with a plunger lift assembly. The valve at the wellhead was closed. The tank battery consisted of one, 100 BBL production tank, a 50 BBL brine tank and a mechanical separator. The base of the production tank oil saturated soil was still not removed or cleaned up. Upon further inspection, I found the dike completely weathered and incapable of containing any substances resulting, obtained, or produced from operation of the well. The tank battery was surrounded by small trees and tall grass. The identification sign has been updated. The lease road was stable and driveable. The following issues need addressed:

1) Remove and properly dispose of the impacted soil within the tank battery. 2) Reinstall the dike around the production equipment.

0433739411 2018-02-02 UrbanProduction Wells No Violations I conducted a compliance notice follow-up for compliance notice 1740074716. During this inspection, the well was in production. The tank battery consisted of a 100 BBL production tank, a 50 BBL brine tank and a mechanical separator. Valves on the separator were open and the chart dated 2/1/18 showed production. The dike around the tank battery has not been rebuilt and the signage has not been updated to reflect the new ownership information. At the time of this inspection, I was not able to observe if the impacted soil within the tank battery has been removed and replaced due to snow coverage. Vegetative maintenace at the well and tank battery locations has been conducted. The lease road was stable and driveable. The following issues need addressed:

1) Replace the signage to reflect the current ownership information. 2) Reinstall the dike around the production equipment.

0672491787 2006-06-09 Production Wells No Violations Case# 06-010 , Sandra Williams . Inspection showed no evidence of a gas leak at this time . I advised complaintant to contact me next time they smell gas . No violations , Resolved at this time .
1014896540 2014-12-16 UrbanProduction Wells Violations Noted I was on location for a status check. During my inspection, I found the well in production. The well was equipped with a plunger lift assembly. The flow line ball valve at the well was in the open position. The tank battery consisted of a 100 barrel production tank, a 50 barrel brine tank and a separator. Upon further inspection, I found the dike completely weathered and incapable of containing any substances resulting, obtained, or produced from operation of the well. The tank battery was surrounded by small trees and tall grass. An identification sign was posted on the production tank; however, the sign had partially peeled off. There were two missing plugs on the brine tank; one on the load line and the other near the top of the tank. The load line at the production tank had the required plug. There was a 7 day production chart inside the separator dated 12/15. The lease road was in stable condition. The well was placed into production prior to House Bill 278. According to our RBDMS database, Big Sky has failed to file an annual production statement for 2013. I called and left a message with Mr. Barr for a return call concerning the condition of the tank battery and well. Big Sky is required to:

1740074716 2016-04-18 UrbanProduction Wells Violations Noted I conducted a status check on this well. The well was equipped with a plunger lift assembly. The valve at the wellhead was open. The tank battery consisted of one, 100 BBL production tank, a 50 BBL brine tank and a mechanical separator. Gas flow through the separator was clearly audible. At the base of the production tank oil saturated soil was observed. Upon further inspection, I found the dike completely weathered and incapable of containing any substances resulting, obtained, or produced from operation of the well. The tank battery was surrounded by small trees and tall grass. The identification sign was not legible and needs replaced. There was a missing plug on the brine tank on the load line. The tank valve on the production tank was missing a plug. The production chart inside the separator dated 2/29/16 showed production. The lease road was stable and driveable. The following issues need addressed:

1) Remove and properly dispose of the impacted soil within the tank battery. 2) Replace the signage to reflect the current ownership information. 3) Reinstall the dike around the production equipment. 4) The well needs shut in immediately per the temporary restraining order issued by the Trumbull County Court of Common Pleas, dated 4/11/2016.
