1544120846 2018-12-04 UrbanProduction Wells No Violations I was on location to follow-up on Complaint # 5818 regarding an abandoned well that could be a potential safety hazard. At the time of my inspection, I found the well surrounded by vegetation. The well was shut in at the tubing gate valve, and the 4.5-inch production casing was shut in at a ball valve. There was a general odor of gas around the area of the well. There was a metal identification sign laying face down next to the well. I turned the sign over and found that the identification sign was incomplete. I called the two phone numbers listed on the sign. One number went to a window company who stated it didnt own a well. The second number went to an unidentified voice mail. The tank battery is overgrown with vegetation and in general disrepair. The battery was comprised of one brine tank, one 100-
1544446816 2018-04-16 UrbanProduction Wells No Violations I conducted an inspection on this well regarding complaint #5818, concerning safety at the tank battery location. At the time of this inspection, I was on location with Howland Fire Department employee, Tim Cavanaugh, who submitted the complaint. The well was equipped with a plunger lift assembly. The gate valve on the tubing and flow line was closed. No domestic line was connected at the well location. There was a partial identification sign located at the well that needed updated to reflect current ownership. The tank battery consisted of one 100 BBL tank, one 50 BBL poly tank and a mechanical separator. Valves on the separator were closed and the 2 inch flow line had been disconnected from the separator. No leaks were observed at the tank battery location or well location. The tank battery was diked but no legible signage was present. The tank battery location was very overgrown with vegetation. The lease access was stable.
