0000646664 2017-08-09 Drill / Deepen / Reopen No Violations I was on location to check the status of the drilling pits and the progression of the rig move off of location. My inspection found that the pits have been sucked down and have 6-7 of freeboard. The drilling rig and most of the accessories have been moved to the Williams 1H & 2H pad (#s 4101 & 4102). This wellhead and the Bates #1H are currently equipped with a reducer and ball valve and are protected by jersey barriers. I found no violations at this time.

0186188994 2017-09-22 Completion Testing No Violations Moore Well Service were working over well trying to unload the well fluids with an air compressor. Compressed air was injected down the 4.5 annulus and out the 2.375 tubing. The well did not unload. Moore Well Services were rigging up to swab the 2.375 tubing with tools in an effort to unload the well. Flow back fluids were to be collected in a 500 barrel tank. There were 16 - 500 barrel frac tanks on location. There two (2) sand transport trucks on location. The pits afforded adequate volume for fluids and a walk around inspection did not reveal any violations during this inspection.
0369766108 2017-10-23 Completion Testing No Violations I was on location to observe various Frac stages of the well. Producers Service Corporation ( PSC) was the stimulation contractor. The lateral was scheduled for 21 stages of treatment. Each stage was 60 in length with five (5) clusters ( 10 shots each, 60 degree orientation ) equally spaced in the stage. The toe prep work was completed on 10/20 17 with the break down pressure reported to be 4600 psi. An acid pad ( 500 gal 15% HCl ) was pumped followed by a slick water pad. Sand concentrations were started at .5 ppg and ranged up to 1.5 ppg. During the stage an increase of the treating pressure reflected issues with sand placement and a sand screen out was encountered. Repeated attempts to flow the sand off the perfs were performed. The downhole pumping rate was insufficient to allow a pump cycle of the perf guns and plug. PCS continued to attempt to clear to casing enough to allow the perf gun to be pumped into position.
0489983227 2018-02-26 Production Wells No Violations The lease road and location was rocked and afforded adequate access to the well and tank battery. The Bates # 2-H was equipped to produce as an electric pumping well. The well was equipped with a Oilwell Model 80 unit. The required I D signage was handwritten on the units and stock tanks. There was a 55 gallon drum ( marked residual waste ), at the wellhead, connected to fluid containment pumpkin on the stuffing box. The 7 to 4.5 annulus had a pop off valve and gauge. The pressure gauge read zero. The tank battery consisted of three (3) 210 barrel stock tanks and one (1) Gas Production Unit (GPU). An earthen dike surrounded the tank battery. Proper spacing was observed between the production equipment. The preliminary restoration was delayed by weather considerations ( per Cary LeShock, U S Energy ). No violations were identified during this inspection.
0497903846 2017-10-05 Completion Testing No Violations Reliance Well Services were on location to serve as the stimulation contractor. Surefire Wireline Services were employed to serve as the perforating and plug contractor. I was onsite to witness Stage 6 of the completion. U S Energy was experiencing problems with the pump down phase due to high pressures and low flow rates. The company man was waiting on orders. I told the Operator to call wen the job was to go forward.

0578183098 2018-09-18 Preliminary Restoration No Violations Status check inspection for Preliminary Restoration. My inspection found the lease road and pad in stable condition. The wellhead is equipped with 2.375 rods and tubing and operating with the aid of an electric powered pumping unit. The stuffing box is equipped with a pumpkin containment system and the containment hood is plumbed to a 55 gallon drum. The casing flow line is equipped with a line heater and the annulus between the 7 intermediate casing and 4.5 production casing is equipped with a 250psi pressure relief valve. Gauges indicate a pressure of 290psi on the tubing and 450psi on the 4.5 production casing. This well produces to a diked tank battery shared with the Bates Unit 1H (PN#24098) and contains three (3) 210 barrel tanks. There is a heat treating, horizontal separator 50 to the north of the tanks and is surrounded by a secondary containment dike. There is a contract pumper currently on location (Tim) and he is transferring production water to a 500 barrel steel tank. I spoke with the U.S. Energy Field Supervisor (Kerry) about the 500 barrel tank and he said that the tank is not permanent and will be removed when they get ahead of the production water. Valves at the wellhead and on the load lines are equipped with locks. Vegetation is well established around the perimeter of the pad and the required identification is posted at the wellhead and at the tank battery. No violations. Preliminary Restoration is approved.

0735477358 2018-01-17 Preliminary Restoration No Violations The lease road graveled and afforded access to the well and tank battery. The well was completed as a electric pumping unit. The wellhead was configured with 7 intermediate casing, 4.5 production casing and 2.375 tubing with rods. The 7 X 4.5 annular space had the required annular pressure relief valve and gauge. The gauge read 0 psi. No leaks were identified at the wellhead. The tank battery consisted of three (3) 210 barrel stock tanks and a Gas Production Unit ( GPU ) separator. Proper spacing was observed between the equipment. An earthen dike surrounded the tank battery. The I. D information was written on the tanks. I spoke with Cary LeShock ( U S Energy )and requested that he included the Township, County and Lot number. The restorative work had been completed, but I could not evaluate the level of compliance due to snow cover.
0797880147 2017-10-30 Preliminary Restoration No Violations The Bates # 2H pad was shared with the Bates # 1H well ( 34-155-2-4098). The pits were closed, but did not have any vegetative growth at the time of my inspection. The two wells on this pad were completed as pumping wells and await the addition of the pumping units. The access road and pad was stoned and accessible. One (1) - 500 barrel Frac tank remained on location. The erosion controls were still effective. The tank battery was built and consisted of three (3) 210 barrel tanks, one (1) Gas Production Unit ( GPU ). The tank battery and GPU were surrounded with earthen dikes which were seeded. The 8.625 annular manifold was not in place at the time of my inspection. The Preliminary Restoration has not met the requirements at this inspection.

1226313189 2017-07-24 Drill / Deepen / Reopen No Violations U S Energy Rig Nine (9) was drilling on air at a depth of 867 in the Ohio Shale. The seven (7) inch intermediate casing ( 80 joints, 24# ) was being unloaded. As previously requested, the dust from the discharge line, was effectively killed by the addition of a diverting splitter on the end of the line and submerged below the surface of the water. There was sufficent pit volume in the blow pit. I requested that the 13.375 to 9.625 annular space be grouted to surface prior to cementing the seven (7) inch casing. A walk around inspection of the air package, mud pump and perimeter did not indicate any violations during this inspection.
1234886090 2017-08-04 Drill / Deepen / Reopen No Violations I was on location to inspect rig down operations for this horizontal Clinton Well. The fluid level in the drilling pits is being lowered and the pit water is being transferred via trash pump to a 500 barrel frac tank. The pits are lined and have plenty of freeboard. I met with the company man and picked up the cement ticket for the 4.5 production casing cement job. US Energy rig #9 will be moving to the Williams Pad today and Monday. I found no violations at this time.

There were 18 - 500 barrel Frac tanks on location. The pits were empty and an inspection of the liners did not reveal any loss of integrity. No violations identified during this inspection.
1314498631 2017-07-26 Casing String Witnessed No Violations I requested from Cary LeShock, due to excessive erosion developed along the lease road, that some measure of erosion control be employed and maintained until the area was vegetated and stable. He said, that he would install silt fencing that day . No violations identified.
1339479906 2017-11-28 Preliminary Restoration No Violations Access to the pad was on a stoned driveway. The pad was rocked and absent of erosional features. The drilling pits were closed, seeded and strawed. The tank battery was built on the west side of the pad. The tank battery consisted of three (3) 210 barrel stock tanks and one (1) Gas Production Unit ( GPU ). All spacing between equipment was in compliance. There were areas on the perimeter of the pad which will require additional seeding and straw. The well were completed as electric pumping wells and are awaiting electrical service. I brought to U S Energy personnel ( Cary LeShock and Harry ) several noncompliant items. The I. D. signage had not been posted and the 7 X 4.5 annular pop off valves were not present during my inspection. The Bates Unit # 2-H did not pass preliminary restoration.
1501251368 2017-07-28 Drill / Deepen / Reopen No Violations The erosional controls requested previously were erected and are functional. U S Energy Rig # 9 was drilling, on fluid, at 3671 MD. Kick off point ( KOP) was 3391with the curve projected landing at 4103 MD. Proposed total depth was 5255 MD. The pit water will be lowered by pumping excess volume into two (2) 500 barrel tanks. The 4.5 production casing was on location. No violations identified during this inspection.
1501614640 2017-08-01 Drill / Deepen / Reopen No Violations I was on location with Trumbull County Inspector Rick Skidmore to inspect the drilling progress of this horizontal, Clinton Sandstone well. US Energy Rig #9 was currently drilling a 6.25 hole on fluid and sliding at a depth of 4627. The kick-off-point (KOP) was at a depth of 3349 and the current rate-of-penetration (ROP) is one (1) foot per hour. The drilling pits were lined and had plenty of freeboard. The location is dry and in good condition. I found no violations at this time.
1590890876 2017-08-11 Drill / Deepen / Reopen No Violations I inspected this location with District Supervisor Steve Ochs. All drilling equipment has been removed. There are two (2) 500 barrel frac tanks at the northwest corner of the pad and this well and the Bates #1H are currently equipped with a reducer and ball valve and are protected by jersey barriers. The drilling pits are open and have plenty of freeboard. I spoke with US Energy and they will be keeping the pits open until completion operations are finished. I found no violations at this time.
