0050740456 2015-10-16 Plug / Plug Back No Violations 82 of 5 casing was retrieved. The casing parted. Tubing was ran down to 82 and circulated. This did not allow the tubing to drop. A pointed bit was ran down on tool to try and make a opening in the casing. The bit came back with damage on the end suggesting it was hitting the edge of the 5 casing. Kilbarger ran a impression block to determine what the condition of the pipe was in. The impression block showed the pipe had a curl. 1 tubing will be ran on 10-19-15 to try and get the tubing past the obstruction.
0172342757 2015-02-05 Production Wells No Violations Compliance notice follow-up. I met Hocking Hills Energy supervisor Doug Kitchen at the well head. The well has a old motor and tank. The site is overgrown with trees. I informed Doug of the need to plug the well.

The tubing was retrieved and a opening tool was ran down to try and open the casing. The casing was opened with the tool. 1 tubing was ran down to 1052. Formation Cementing on site at 1700 hours. 40 sack bottom plug set. 100 sack plug set 400 to surface.

0784671462 2015-11-25 Final Restoration No Violations Final restoration inspection. The well has been plugged. The well site has vegetative growth. The roadway does not have vegetative growth. Flow line remains. I have spoken with well owner Doug Kitchen concerning the remainder of the restoration. He stated the flow line would be removed and the roadways seeded by 2-1-16.

0871707182 2014-04-10 Production Wells No Violations This is a follow-up inspection for the well being idle. Andy Thomas and Kip Thompson inspected this well before and wrote it up for not the well not being able to produce. The violation was issued on 1/11/2012. The well is still idle and there has been no attempt to put the well back into production. I will contact Kip Thompson and let him know about the situation.

1335741100 2018-01-18 Plug / Plug Back No Violations On the day of this follow up inspection, I found that all the pipe, electric lines and parts from the wellhead are still on site with no changes. I called Hocking Hills Energys Doug Kitchen about the clean up and reminded him that remediation was past due and Doug told me soon as the snow melts down and they finish the well there are drilling he would send out his crew to clean up the remaining remnants of the well site.

1444920531 2015-10-15 Plug / Plug Back No Violations Kilbarger rig 7 has pulled the tubing and rods. No information regarding the casing program. 1173 of tubing was retrieved.

1507727095 2017-10-11 Production Wells No Violations On the day of this inspection, I found an oil and gas well that has been plugged. I walked around the well site and found a flowline and electric power wire on the ground. I also found the top of a wellhead that was down the hill from well site that was not attached to anything. Talked to Hocking Hills Energys Doug Kitchen he said that they would have the well site cleaned up in the following two weeks from the time of my inspection.
1510060420 2017-11-07 Final Restoration Violations Noted On the day of this inspection, I found oil and gas well that had been plugged. The plugged well site still has piping and an electric line with remnants of a pumping unit that is down hill of the well site at time of my inspection. Oil and gas site restoration did not meet 1509.072B requirements at time of my inspection. I talked with Doug Kitchen of Hocking Hills Energy, Doug said he would get the well site in compliance and would call me soon as piping and other remnants were cleaned up.

There are pumping remnants on the ground and there are rods and tubing in the well. Electric wires and box present, but are laying on the ground. There has been no resolution to any of the issues. I talked to Dave Poling with Hocking Hills Energy and explained the issues. He will work with the Vinton County Inspector, Kip Thompson, on a timeline to plug or produce the wells.
