Tank area good, well approx 1/2 mile back in woods, ID corrected on tank. No violations, could probably not have been prevented
1006331468 2015-04-02 Production Wells No Violations Upon inspection, it was observed that this well is capable of production, but is currently shut in. The well consists of a pumping unit standing over 4.5 casing and 1.5 production tubing. I spoke with EnerVest Well Tender Mr. Jim Hughey and he stated that this well will be plugged this spring.
1088724404 2016-05-25 Final Restoration No Violations The purpose of my inspection today is a status check on a final restoration. During my inspection I was accompanied by ODNR Inspector LJ Parkins. The inspection revealed that there was no evidence of an oil and gas well and the site has been restored. Location has met 1509.072 B requirements.
1567784552 2015-08-06 Plug / Plug Back No Violations On location to witness plugging operations for this well. Upon arrival, Appalachian Well Surveys was rigging up to perform a bond log on the 4.5 casing from 2500-1400. The bond log showed undocumented perforations across 1 joint of casing and thru the surface casing from 1404-1448. The perforated zone had cave in on the 4.5-8.625 annulus, hanging the casing in the hole. AWS attempted to shoot the 4.5 casing off at 2052, but the casing could not be pulled. Chipco Well Services ran 1.5 tubing into the hole and set a 250 bottom of surface casing plug through the holes created by the shot. Tubing was pulled OOH and the 4.5 casing was then ripped at 1150. Chipco then set a 150 plug to isolate the ripped casing and newly discovered perforations. The remaining 4.5 casing was pulled to 200 and fluid was measured at approximately 200 down. It was then decided to pull the remaining casing OOH and fill the 8.625 casing from surface to create the surface plug. Circulation was established for all plugs except the surface.

2117725142 2015-08-04 Plug / Plug Back No Violations On location to witness plugging operations for this well. Chipco Well Services LLC is rigged up over the well and 1.5 tubing is set at 5236. Chipco Well Services pumped a 400 bottom plug using 33 sks 50/50 poz cement and 12 bbls of freshwater displacement. Tubing was then pulled to 3829 and a 200 Big Lime plug was set using 63 sks 50/50 poz cement and 9 bbls of freshwater displacement. The Big Lime plug was set through 10 perforations made by Appalachian Well Surveys from 3820-3830. Tubing will be pulled OOH and the 4.5 casing will be ripped at the free point.
2121221987 2015-08-05 Plug / Plug Back No Violations Received call from Chipco Rig Operator stating that telegraph could not be found during attempts to rip the 4.5 casing. Upon arrival on location, Chipco was attempting to rip just below the bottom of the surface casing at 1875 with no success. It was decided to bring Appalachian Well surveys in tomorrow morning to run a bond log and determine the next course of action for plugging.
