0997787721 2005-02-25 Orphan No Violations Inspection this day found well right along side the road, well sticking way up in air and tied into steel line going ?, 4 1/2 casing appears to have hole in it at ground level, probably 5 between ground level and wellhead that looks like big washout, no IDs, has not been produced in long time, probably orphan well that state will have to plug. No hazard.

1367342765 2013-04-30 Orphan No Violations My inspection this day to do an up-date on condition of well and complete new SEM. Today, found that casing that was sticking up in air, in center of a large hole that has been washed out, has been broke off or has disappeared - no sign it of where the hole was. Deep indention is filled with leaves and a tire at the center bottom. Got down in hole but not sure how far down it is washed out. Losing this casing out of the well will make it harder to plug. Pictures taken, new matrix is 151.

Larrey Martin, familiar with the area and well, believes there is a swab stuff in the 2 tubing in the hole. He says the well continued to sell gas for years after that. This SOW and BID should be reviewed and corrected to include the work needed to drill out or fish out this steel and plug the well. Also met with Mrs. Arnold.
1455639975 2016-02-16 Administrative Inspection No Violations Based on a Lexis Nexis search for Patty or Patricia Martin, who owns a mineral interest in the land this well is located, I have called 330-852-2911 and left a message (I do not believe this is the correct Patricia Martin) with no response , 330-922-9461 which has been disconnected, and 410-832-5585 (tried a total of 4 times this day), which lists a Marietta address and which rang through but there was no voice mail or message option to leave. I also called 740-366-5177, in Newark, OH and 724-569-1319 in Smithfield, PA and they were not Patricia Martin. On 2/17/16, I again called 410-832-5585 and got no answer, could not leave message. I believe we have exhausted the phone numbers available for Patricia or Patty Martin with no success. Possibly a letter to the most recent address would prove a better option of locating here.

2072037951 2007-10-18 Orphan No Violations Inspection due to request from Forest Service of a well they would like plugged. Well sits just off the west side of Twp. Road #381, about 15, and there is a deep cavity around the well casing. Cant get close enough to the casing to measure it - it is a danger to any one playing in the area. Casing, I believe, is 6 1/4 and sticks up above ground level about 7-10 - ground is hollowed out around it and logs have been laid across the hole to be able to stand on to work on the well. No leaking gas, but this is a party spot for teens - reportedly often beer drinking and someone could easily get hurt. While well is just off the side of the road - well head is about level with the road, location will have to be built cutting the bank down about 10 and may be difficult to do without causing a slip along the road. Pictures can not show how deep the cavity around the well is. There is 2 steel running away from the well - this big lease had 4-5 wells on it originally - Carl Heinrich owns some of the other wells but has never produced - and suspect they were all tied together at one time with this steel line. Old map shows this as a Berea gas well but no records have been found.
