1201121279 2008-01-23 Orphan No Violations Operator drilled approximately 60 feet when water started discharging from hole. Placed 80 feet of plastic 5 inch pipe in 8 inch hole. Cemented between 8 inch and 5 inch casing.. No water discharging from hole at time of inspection. Will return to resume drilling to clean out well.
1202923724 2008-02-13 Orphan No Violations Checked well at request of operator. Operator has been working on well for several weeks. Well was cleaned out to depth 588 when bit became stuck and the 2 inch tubing broke at 195 feet. A tool was used to try to retrieve the remaining tubing and bit but only 2 joints came out. A down hole camera was used and showed debris lodging in the 6 inch hole when pulling out from hole. Operator will run 1.5 inch tubing into hole to top of 2 inch tubing and try to run 1.5 inchtubing down outside of 2 inch tubing to depth 588. Cement will be pumped into well to plug well. Discussed procedure with supervisor Jeff Fry.
1203005299 2008-02-14 Plug / Plug Back No Violations Operator ran 1.5 inch tubing into 6 inch well and hit 2 inch tubing stuck in well at approximately 237 feet. Could not get 1.5 inch tubing past 2 inch in hole. During drilling, when drill string broke, 9 joints of 21 foot pipe were retreived along with 5 feet of the broken section. After fishing for the 2 inch string an additional 2 sections of pipe were recovered, but the remaining piece of broken pipe was not found. Bob thinks that when the drill string broke that the remaining string on the drill continued down past the broken 2 inch in hole. The broken section of the 2 inch caught next to the 2 inch in the hole. Bob thinks the broken section of 2 inch prevented the 1.5 inch tubing used in reverse grouting to penetrate past the top of the remaing string in the hole.
1203446262 2008-02-19 Administrative Inspection No Violations Voice mail message from Bob Harpster on 2/15/08 indicated that cement in well dropped 60 feet when check in the morning. Three bags of cement were mixed to fill the well. The casing was cut off 4 feet below ground elevation, ID plate attached and pit backfilled.
1207165445 2008-04-02 Final Restoration No Violations Checked site for final restoration. All equipment removed. Farm field in good condition, no significant erosion/rutting. Small pile dirt on top of well site. Well cut off below ground elevation. Final restoration acceptable.
