A three inch transfer line from the separator has been replaced with schedule 80 PVC pipe. A two inch transfer line near the base of the separator tower has a slow leak. The concrete pad for the tower separator is deteriorating and the integrity is in question. The horizontal separator has broken sight tube fittings and the two inch change over from PVC to steel is leaking. Free standing oil is present in the dike. The operator is advised to utilize a steam pressure washer to clean the liner and remove the oil. The operator is advised to replace all flow lines in the containment area and remove any equipment that is not required. The operator may consider placing the tri-plex pump onto blocks within the dike area to prevent future flooding of the pump and motor. The operator shall notify the DOGRM prior to initiating production of producing wells and the injection well.

1239981045 2016-05-10 Surface Facility Construction No Violations Pressure up recently installed three inch flow line to test for leaks. Previous leaks were reportedly repaired. Upon arrival at location the operator pressured up the line for testing and shortly thereafter a fitting failed. Pressure testing of the line did not occur due to the failure. The operator determined while the installation of the pipe line occurred the banks of the trench collapsed onto the pipe before the glue fittings were completely cured. This has caused the fittings to repeatedly fail at various locations. The attempt to use this pipe for water discharge will not occur. The intention will be to insert a smaller diameter pipe through the 3 inch pipe as a conduit to the well.

The treatment caused the iron sulfide and rust scale to plug off tubing at about 600 feet below grade. The operator had a service rig on the well and was attempting to clean out the 2 7/8 inch upset tubing with a 2.5 inch cable tool drill bit and bailer. The scale buildup started at 300 feet below grade. The operator will be required to conduct a mechanical integrity test on the annulus when this operation is complete.

The operator was advised the accumulation of rain water within the dike area needs to be transferred into the salt water storage tanks for disposal in the salt water injection well.

The tri-plex pump discharge nipple failed and the discharge line sheared off from the pump. This failure caused the contents of the two brine storage tanks to release into the dike containment area. The fluid was contained in the dike containment area. The operator discovered the on Friday July 30, 2016. The operator pumped the water into the storage tanks and emptied the dike containment area. 1654306375 2017-08-29 Salt Water Injection Well No Violations Service rig is on well to clean out blockage of iron sulfide and rust scale in the 2 7/8 inch tubing. The operator used a 2.5 inch cable tool drill bit and 2 inch bailer to drill out the obstruction. Upon arrival the operator was tripping out of the hole after reaching the bottom of the tubing and the hole. The operator measured the fluid level with a digital echo meter. The fluid level in the annulus was measured at 121 feet below grade and the tubing was measured at 464 feet below grade. The annulus was pressure tested for integrity because of the clean out activity. A 230 cubic foot bottle of nitrogen was used to pressure the annulus to 85 psi and monitored for more than 15 minutes with zero pressure loss. A second 230 cubic foot bottle of nitrogen was used to increase the pressure to 160 psi and monitored for 15 minutes with zero pressure loss. The operator was given authorization to resume injection operations.

2047264810 2016-04-26 Surface Facility Construction No Violations The operator installed Harvel PVC 1120 at 3 inch Schedule 80 ASTM D 1285 pipe rated to 370 psi at 73 degrees Fahrenheit. Twenty one 20 foot sections were installed and connected with connections glued with cement. The line is about 420 feet long. The operator buried about half the line. The first line test was on April 20th. The test failed due to a line leak near the well head at the riser to the surface. The leak was repaired and the steel riser pipe was replaced with a PVC pipe. The flow line was loaded with water and the line was pressure tested. The test pressure to attain was 300 psi. While pressure the line at about 280 psi a fitting failed and the pressure rapidly dropped to about 20 psi. During this testing process 20 psi was not exceeded and the 600 psi rated shut off valve catastrophic failed. The valve was replaced with a second new valve. The test pressure was not achieved. The operator was tasked with locating the leak.
