0783244578 2009-05-04 Orphan No Violations Checked site from oil spill reported 5/1/09. On 5/1/09 inspectors Tom Benko and Sam Faust responded to an oil spill into a ditch in Wood county, Middleton township, section 26. Oil discharged from a field tile into a ditch along State Route 582. The well, located in a farm field, was found cut off several feet below the ground. An oil stain on the surface of the ground helped locate the well. An open 4 1/2 inch casing was noted cemented inside a 9 inch casing. A 4 inch diameter PVC pipe approximately 8 feet long was cemented inside the open casing to contain the discharge from the well. Oil booms were located in the ditch at the time of the inspection on 5/4/09. There was a slight oil sheen on the water discharging from the tile into the ditch. Spoke to USEPA inspector Brian Kelly by phone. Federal funds from US Coast Guard will be sought to pay for plugging well since drainage from ditch flows to navigable waters.
0958647943 2009-05-18 Orphan No Violations Reviewed aerial photographs at ODNR SWC and US Dept of Agriculture office to try to identify well in farm field. Checked aerial back to 1980 but could not see signs of well in field. Maps are of large scale and a well may be difficult to see on the maps.

There does not appear to be a significant increase in the amount of oil in the ditch from inspection on 5/4/09. Absorbent booms are still in place and discharge continues from tile. Spoke to landowner Don Limes by phone and recommended that lime and peat moss be tilled into area around well head where oil is on ground. Farm field appears to have been recently planted.
1242996651 2009-05-01 Production Wells No Violations Contacted by OEPA regarding a release of crude oil into the waterway. Upon investigation a 9 inch casing with 4.5 inch casing was uncovered. And cut off 6 inches below the ground. Oil was present in borehole about 20 to 30 feet below grade. An unregistered producing domestic gas well is located 0.5 mile south shows 150 lb of pressure.

If booms are not removed before the contractor plugs the well, the contractor will be instructed to remove and properly dispose of the booms as part of the plugging contract.
1259173999 2009-11-25 Orphan No Violations Bradner Oil moved on to site and has exvacated around the well casing. A 12 inch steel pipe approximately 6 feet long has been driven into the ground around the well. When operator moved on to site 11/23/09, oil had overflowed from the PVC riser pipe extending from the well casing. Some oil was pooled on the ground around the well. Check of ditch during inspection found no oil discharge into the ditch. The contaminated soil was segregated and covered with plastic. The 12 inch pipe was cemented around the 8 inch conductor pipe on the well for wellhead control, however oil began leaking outside of the 12 vinch pipe. Decision was made to run 40 feet of 4 inch steel pipe with packer at bottom, inside of the 5 5/8 inch surface casing. There is an obstruction in the well approximately 40 feet deep. Check of well logs shows depth to bedrock 30-40 feet in the area.
1259851460 2009-11-30 Orphan No Violations Nobody on site at time of inspection. Drill rig not yet setup on well. Noted some additional oil accumulated in pit. No discharge of oil from field tile into ditch.
1260460448 2009-12-07 Plug / Plug Back No Violations Operator pumped approximately 70 barrels of oil and fluid from well. Oil was removed from site by vac truck from Roger Frantz. Fluid in well was replaced with brine water due to gas pressure in well. Well was cleaned out to depth of 1065 feet. Two inch tubing was run into well to depth of 1050 and 125 sacks of cement were pumped into well. Water in well was circulated to tank. Cement set up at depth of 200 feet. On 12/8/09 inspector Sam Faust witnessed reverse grout with 9 sack grout to surface of well.
1262098070 2009-12-21 Final Restoration No Violations Checked site for Final Restoration. Well has been plugged and cut off below ground. All equipment removed and farm field rough graded. Site will be recommended for final restoration.
1945333877 2009-12-01 Orphan No Violations Operator on site. Rig set up on well. Ran 40 feet of 4 inch steel pipe into well with packer on bottom to control oil discharging around outside of well casing. Will start drilling in the morning.

Petroleum odor at well may be from oil contaminated soil or gas from well. Plugging specs for well being developed and contractors will be contact to submit bids to plug well.
