1840728 Primary Facility 2009-11-03 Routine/Complete Inspection No Violations Noted Drilling had been completed at the time of the inspection. Drill rig was in the process of being moved off-site. Below grade pit located onsite and it had more than the required two-feet of lined freeboard. Observed no accelerated erosion & sedimentation in relation to the site or access road. The Department recommends that the operator implement addiotional rock on the access road and well-pad if any well stimualtion activities are undertaken to minimize the amount of sediment generated and transported onto Township Road 714.
1856213 Primary Facility 2010-01-06 Routine/Complete Inspection No Violations Noted Drilling had been completed at the time of the inspection, well appeared to be equipped for production. Site had been regraded. Brine tank located onsite and appeared to be in sound condition. The operator should monitor the site as the weather improves for accelerated erosion & sedimentation and to insure that the vegetative cover meets the 70% uniform coverage criteria.
1981732 Primary Facility 2011-06-20 Site Restoration No Violations Noted Drilling was completed on 11/02/09. Well appeared to be equipped for production. Brine tank located onsite and appeared to be in sound condition. Observed no accelerated erosion or sedimentation in relation to the access road or well-site. Noted that the well was located in what had been a cultivated cornfield.
2559934 Primary Facility 2017-02-07 Routine/Complete Inspection No Violations Noted Well tagged-shut in. Annulus shut. A check with efacts disclosed production reported for previous year. Well head casing-equipment-50 bbl fiberglass tank looked ok.
