1883413 Primary Facility 2010-05-03 Routine/Complete Inspection No Violations Noted Drilling appeared to have been completed at the time of the inspection, cable rig located onsite at the time of the inspection. Below grade pit located onsite and it had more than the required two feet of lined freeboard. Observed that the culvert implemented to carry the water flowing from the collection ditch at the back of the cut slope had not been installed correctly. Specifically the culvert outlet was approximately 18 inches above the surface of the ground and the outletted water flowed onto disturbed soil. There were hay bales placed in the outlet flow, however there was more disturbed earth below the hay bales and the flow appeared to be going directly under the hay bales. Contacted operator representative Jeff Grove regarding this matter and he stated that he will visit the site today and determine the best course of action to address the problem, and that work to do so will begin as soon as possible.
1883697 Primary Facility 2010-05-12 Routine/Complete Inspection No Violations Noted Drilling appeared to have been completed at the time of the inspection, cable rig located onsite at the time of the inspection. Below grade pit located onsite and it had more than the required two feet of lined freeboard. Observed that the culvert implemented to carry the water flowing from the collection ditch at the back of the cut slope had not been installed correctly. The culvert had been cut back but the stabilized rock outlet had not as of yet been implemented due to the fact that site restoration activities had not as of yet been undertaken. Noted that due to recent precipitation events sediment had been generated but had not been transported off-site. The operator should begin site restoration activities and address the culvert issues as soon as possible.
1898180 Primary Facility 2010-07-14 Site Restoration No Violations Noted Drilling had been completed, well appeared to be equipped for production. Brine tank located onsite and appeared to be in sound condition. Site had been regraded, seeded, and mulched. Vegetation ad begun to grow, but did not as of yet meet the 70% uniform coverage criteria. The operator should continue to monitor the site and if necessary reseed and mulch the site to achieve 70% uniform coverage. Observed no accelerated erosion & sedimentation in relation to the site or access road.
1992536 Primary Facility 2011-06-29 Site Restoration No Violations Noted Drilling was completed on 04/26/10. Well appeared to be equipped for production. Brine tank located onsite, and it appears to be in sound condition. Vegetative cover met the 70% uniform coverage criteria and no accelerated erosion or sedimentation was observed in relation to the access road or well-site.
2215205 Primary Facility 2013-10-18 Routine/Complete Inspection No Violations Noted Well appeared to be equipped for production. Brine tank located onsite and it appeared to be in sound condition. There was no observed accelerated erosion or sedimentation in relation to the site or access road.
