1983996 Primary Facility 2011-06-27 Complaint Inspection No Violations Noted Well stimulation activities were ongoing at the time of the inspection. Spoke with pad manager Rick Olson regarding the water that had been observed on Penn View Road. Mr. Olson stated that the water is river water that is being withdrawn from the Allegheny River and deposited onto the paved roads to address dust issues. Mr. Olson stated that initially he questioned the water on the road and he contacted his superiors at EXCO and inquired about the watering of roads. He was informed that they were indeed spreading river water to minimize dust in the residential areas. This practice had been suspended during rainy days, however was begun again recently. Observed no accelerated erosion or sedimentation issues while onsite, however a full site inspection was not performed due to the ongoing well stimulation activities.
1987403 Primary Facility 2011-07-11 Incident- Response to Accident or Event Violation(s) Noted Inspection performed as a result of the operator notifying the Department of a release of flowback fluids to the ground. Wells were actively being flared at the time of the inspection. According to Doug Byers, who WQS Higgins met onsite an out of service pipe was left adjacent to the flare trailers brought onsite. This pipe still contained flowback fluid. Upon ignition of the flare stacks the fluid in the pipe was heated resulting in the pipe bursting and releasing the flowback fluid to the well pad immediately adjacent to the flar trailers and to a stormwater management trap. A vacuum truck located onsite vacuumed any fluid left on the surface. Prior to excavating the affected fill area the flare stacks had to be safely moved to another section of the well pad. Once this is completed the affected soil will be excavated and sampled to insure that all of the contaminated media had been excavated.

Brine tanks located onsite and they appeared to be in sound condition. Noted that the roll-offs boxes that had contained the contaminated mdeia excavated as a result of the flowback discharge had been removed. There was no observed accelerated erosion or sedimentation observed in relation to the access road or well-site. Noted that the public road leading to the well pad had been repaved. Site required final stabilization, i.e. final grading, seeding & mulching. The Department recommends that the operator undertake temporary erosion controls to insure continued site stabilization through the winter months.

The well integrity appeared to be in good condition, with no leaks or spills. Only some minor rust was observed. Flow could be heard. The cellar was filled to surface with gravel. There was a tubing gauge that was reading 204 psi. The well was properly labeled and the Emergency Response sign was posted at the entrance to the site. There were 4 tanks contained within a lined steel dike. 2 dual line heaters were also being used.
