1947285 Primary Facility 2011-02-02 Drilling/Alteration No Violations Noted Drilling 8 3/4" hole at 1440' at time of inspection. Proposed 1470' of 7" .250" wall casing to be set. Information per drillers log and cement tickets: 74' of 20" conductor set. 632' of 9 5/8" casing set and cemented with 350 sacks of "standard" cement containing 3% CACL2 & 1/2# flake (yield=1.18 cu ft/sk). Cement returns to surface not realized with 30' of cement left in hole at 20:11 01-31-10. Grouted backside with 9 bbls of cement. Cement basket set at 378' as proposal indicated potential mine at 406'-436'. Open mine not encountered. Drilling pit lined and freeboard maintained. E/S controls in place and functioning as per available plan.

Brine tanks and seperator apparatus located onsite and appeared to be in sound condition. There was no observed accelerated erosion or sedimenation in relation to the access road or well-site. Site and freshwater impoundmnet had been regraded, seeded and mulched. The oprator should continue to monitor the site to insure that 70% uniform coverage criteria.

The Department suggest that the Operator line the ditch at the bottom of the hill adjacent to the well head with clean durable rock. The Department also suggest the Operator regrade the affected area and install rip rap and catch basin between the well pad area down to the sediment fencing installed at the limit of disturbance.
