Inspection Data

Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
2066767 Primary Facility 2012-05-10 Routine/Complete Inspection No Violations Noted Site construction was underway at the time of the inspection. Fresh water pit was located onsite and it had more than the required two feet of lined freeboard. Noted that the interceptor ditch shown on the E&S plan should be rocked or stabilized with erosion control matting. In addition the use of hay bales at the outlet of the interceptor ditch is not an appropriate use of this BMP. The operator should implement a rock lined outlet or a small rock lined sediment trap. At this time the Department is requesting that the operator address these minor deficiencies with the E&S plan and its implementation and forward documentation that these issues have been addressed no later than May 28, 2012. This information should be sent to the attention of Water Quality Specialist Greg Higgins at the above address.
2068091 Primary Facility 2012-05-15 Drilling/Alteration No Violations Noted @ 420'; Notif OK
2069491 Primary Facility 2012-05-17 Drilling/Alteration No Violations Noted waiting on BOP; Intermediate installed
2072300 Primary Facility 2012-05-25 Routine/Complete Inspection No Violations Noted Drilling was ongoing at the time of the inspection. Dril pit and freshwater impoundment located onsite. Both had more than the required two feet of lined freeboard. E&S plan onsite and the plan has been implemented properly. There was no observed accelerated erosion or sedimentation in relation to the access road or well-site. The operator should continue to maintain the site and road erosion controls until well development activities are completed.
2081673 Primary Facility 2012-07-09 Routine/Complete Inspection No Violations Noted Drilling had been completed at the time of the inspection. Drill pit and freshwater impoundment located onsite. Both had more than the required two feet of lined freeboard. E&S plan onsite and the plan has been implemented and maintained properly. There was no observed accelerated erosion or sedimentation in relation to the access road or well-site. The operator should continue to maintain the site and road erosion controls until well development activities are completed.
2095772 Primary Facility 2012-08-28 Routine/Complete Inspection No Violations Noted Drilling had been completed, well appeared to be equipped for production. Brine tanks located onsite and they appeared to be in sound condition. The operator should insure that the containment diking constructed around the brine tanks meets the permeability requirements to insure that it effectively contains any fluids should a release occur. All buried metallic piping related to the conveyance of production fluids to the brine tanks should mee the corrosion protection requirements set forth in OGA 3218.4(a). The site had been regraded, however the topsoil had not been spread over the well pad. The well site was not temporarily stabilized as it should have been as per 25 PA Code 102.22(b). The Department recommends that the operator undertake temporarily stabilization if the remainder of site restoration actvities are not to be completed in the immediate future. Frac impoundment located onsite and it had more than the required two feet of lined freeboard.
2142916 Primary Facility 2013-02-26 Routine/Complete Inspection No Violations Noted Well appeared to be equipped for production. Brine tanks located onsite and they appeared to be in sound condition. Site had been regared and temporarily stabilized. The was no observed accelerated erosion or sedimentation in relation to the access road or well-site. The operator should continue to monitor the site to insure that the vegetative cover attains the minimum 70% uniform coverage.
2244573 Primary Facility 2014-02-11 Routine/Complete Inspection No Violations Noted Well appeared to be equipped for production. Brine tanks located onsite and they appeared to be in sound condition. Site was snow covered.
2650681 Primary Facility 2017-10-23 Routine/Complete Inspection No Violations Noted Well appeared to be equipped for production. Brine tanks located onsite and they appeared to be in sound condition. There was no observed accelerated erosion or sedimentation in relation to the site or access road.
2684915 Primary Facility 2018-01-22 Routine/Complete Inspection No Violations Noted At time of inspection, well drilled and appears equipped for production.
2827865 Primary Facility 2019-01-16 Routine/Complete Inspection No Violations Noted On 1/16/19 at 0829 hrs., I conducted a routine inspection of Redmill Drilling's H M Boyer 3-77 well (API 005-30920) in Redbank Twp., Armstrong Co. The well pad is restored and permanently stabilized. Two brine tanks are located within adequate secondary containment. The well is equipped for production. A site ID sign is present at the entrance to the well pad. A well tag should be affixed to the well head. There are no violations being cited for this inspection.
