2668361 Primary Facility 2017-12-06 Routine/Complete Inspection No Violations Noted Site had been constructed as outlined in the E&S plan contained within the ESCGP. All site and access road erosion controls had been implemented and properly maintained. There was no observed accelerated erosion or sedimentation in relation to the site or access road. All disturbed areas had been stabilized with vegetation attaining 70% uniform vegetative cover. The well appeared to be equipped for production. There were two brine tanks located onsite within containment. Both the tanks and containment appeared to be in sound condition. There was one Baker tank and four sealed roll-off boxes located on containment on the gravel well pad. Additionally there were four roll off boxes with secondary containment located on the AST pad that appeared to contain pieces of secondary containment. It was also noted that pieces to the AST were located onsite on what had been the AST pad. Finally there was a well development impoundment located onsite and it appeared to contain freshwater.
2698840 Primary Facility 2018-02-27 Routine/Complete Inspection No Violations Noted Site and access road had been constructed in accordance with the ESCGP. All site and access road erosion controls had been implemented and maintained properly. All disturbed slopes and soil stockpiles had been stabilized with vegetation. There was no observed accelerated erosion or sedimentation in relation to the site or access road. It was noted that significant precipitation events had recently ceased. Two produced fluids tanks were located onsite within secondary containment. Both the tanks and containment appeared to be in sound condition. Well development impoundment located onsite and it appeared to contain collected precipitation and withdrawn fluids. There was an Act 9 sign located onsite in accordance with 25 Pa Code Chapter 78a.55(i).
