THE SITE IS IN GOOD CONDITION.

2570672 Primary Facility 2017-03-09 Drilling/Alteration No Violations Noted TOP HOLE DRILLING IS COMPLETE. THE SITE IS CLEAN AND PROTECTED FOR THE BIG RIG. THE WELL IS COVERED.
2624537 Primary Facility 2017-08-03 Routine/Complete Inspection No Violations Noted On8/7/2017, WQS Brian MacQuarrie onsite. No current activity on well site. Four wells drilled. Large AST used for well stimulation setup onsite. Secondary containment setup around wells. Six frac tanks onsite within secondary containment structure. Previously disturbed areas well vegetated.
2672170 Primary Facility 2017-12-18 Routine/Complete Inspection No Violations Noted Site and access road had been constructed in accordance with the E&S Plan contained within the ESCGP. All installed erosion controls had been properly maintained. It was noted that on several slopes along the cut side of the access road that several areas were lacking vegetation. Weather permitting it is recommended the operator evaluate and take any action necessary to address these areas. There was no observed accelerated erosion or sedimentation in relation to the site or access road. Work was underway to complete well development and equip the well for production. Three Baker tanks were located onsite within containment. Constructed AST was located onsite for the containment of flowback fluids. Registered well development impoundment located onsite and it contained fluids.
2695382 Primary Facility 2018-02-16 Routine/Complete Inspection No Violations Noted Site had been constructed as outlined in the E&S plan contained within the ESCGP. All cut and fill slopes for the access road and site had been temporarily stabilized with vegetation. All sediment traps and silt socks appeared to have functioned properly in dealing with runoff as a result as excessive precipitation. There was no observed accelerated erosion or sedimentation in relation to the site or access road. Wells were in the process of flowing back and being equipped for production. Two brine tanks were located onsite within secondary containment. Both the tanks and containment appeared to be in sound condition. Constructed AST was located onsite. Well development located onsite and it appeared to contain withdrawn water and accumulated precipitation. There was an Act 9 sign located onsite in accordance with 25 Pa Code Chapter 78a.55(i).
2844057 Primary Facility 2019-02-25 Routine/Complete Inspection No Violations Noted At the time of inspection well equipped for production. Brine tank within secondary containment and appears structurally sound. GPU within secondary containment. No E&S issues.
