2654228 Primary Facility 2017-10-30 Routine/Complete Inspection No Violations Noted On 10/27/2017, WQS Brian MacQuarrie met onsite with Joseph Schwab -Penn Energy for the purpose of conducting a pre- operation inspection. Well permits and ESCGP onsite. All erosion and sedimentation best management practices installed. Site has been stabilized with seed and mulch. No apparent E/S issues.

As a result of this inspection, drilling activities may commence.

2709169 Primary Facility 2018-03-23 Drilling/Alteration No Violations Noted A 16" conductor casing was set at 40' and cemented up into the 8'X8' cellar.
2709969 Primary Facility 2018-03-26 Routine/Complete Inspection No Violations Noted Site and access road had been constructed in accordance with the ESCGP. All site and access road erosion controls had been implemented and maintained properly. All disturbed slopes and soil stockpiles had been stabilized with vegetation. There was no observed accelerated erosion or sedimentation in relation to the site or access road. Drilling had commenced but was not ongoing at the time of the inspection. Work was underway onsite for installation of the sound barrier wall to be located onsite. There was an Act 9 sign located onsite in accordance with 25 Pa Code Chapter 78a.55(i).

Seven roll-off frac tanks on-site within secondary containment. Materials and equipment stored on-site within secondary containment. One AST on-site with signage identifying contents. One of the three AST inflow/outflow valve locations did not have additional spill containment structures. Department recommends Operator install spill containment structures beneath inflow/outflow valves. No E&S issues.
