2554625 Primary Facility 2017-01-19 Routine/Complete Inspection No Violations Noted I arrived at 12:30 to conduct an inspection of the site. Helmerich & Payne Drilling Rig #513 is the drilling rig at the location. The pad surface contained a significant amount of mud due to the recent rainfall events and snow an ice melt. Chief submitted a Drilling Resumption After Break of 30 days or more notification as per 58 Pa. C.S. 3211(f)(1) to the Department on 1/17/17. The notice indicated Chief would resume drilling of the 3H well at 6:00 Pm on 1/18/17. I met with Trey Villareal, site company man, consultant of Chief Oil and Gas and Ray Miller, Senior Health Safety and Environmental Advisor of Chief Oil and Gas. The current drilling depth is 0 feet. According to Mr. Villareal and Mr. Miller resumption of drilling has not commenced on the 3H well as of yet. This rig has been stacked out for 15 months in Texas and is currently undergoing winterization and rig test up. Drilling will commence on Sunday. Vegetative cover on the site is greater than 70%. Several areas along the access road and adjacent to the well pad have been disturbed by vehicles parking on the soft ground. These areas have been smoothed and mulched with straw. No evidence of accelerated erosion or sedimentation was observed.

Drilling operations are on-going. All chemicals and equipment on site were being stored within containment, and no observable leaks or spills were present. No evidence of spills such as sheens or odors were observed on the areas of the pad outside containment.

Chief representative Colter Dabney was on site and accompanied me on the inspection. He is aware of the puddle in containment with the petroleum sheen and the hydraulic fluid spill, and indicated that these items would be addressed.

2587297 Primary Facility 2017-04-26 Routine/Complete Inspection No Violations Noted The 2H is having the coil tubing run down hole. The site Forman is Dale Graham. The Frack is being conducted by Universal. No leaks from equipment. Containment is in good condition. No violations noted.
2607090 Primary Facility 2017-06-05 Routine/Complete Inspection No Violations Noted I arrived at the site at 12:13. The weather conditions were mostly cloudy to partly sunny skies, humid with a light breeze and temperatures in the low 70's. I met with Larry Patrick and an employee with the first name of Gary, who declined to provide his last name, both well testers of Oil States Energy Services. They stated they following in reference to site activities. "Flowback of the 2H and 3H wells is in progress. The flowback operations started last Wednesday or Thursday. The 1H well is in production. Natural gas is being sent to the gathering pipeline on the pad to the sales line as the wells are being flowed back." I left the site at 13:35.

I contacted Peg Farrell, Environmental Manager of Chief Oil and Gas at 14:13 on 6/5/17 to inform her of my findings at the site during the inspection and to confirm the statements of Oil States Energy regarding site activites. Ms. Farrell indicated during the phone conversation that the containment at the site was inspected the week of 5/30/17 and the data was

2751410 Primary Facility 2018-06-25 Routine/Complete Inspection No Violations Noted I arrived at the site at 12:59. The weather conditions are partly cloudy skies and breezy with temperatures in the low to mid 70's. The entrance gate to the pad on the access road was unlocked. There was no activity or personnel at the site. The L&L Construction Unit A 1H-3H wells are in production. The most recent inspection conducted at the site by the Department was on 2-16-18, with the inspection code listed in the surface activities inspection report as Site Restoration. The well pad has been restored. The Department has acknowledged the Notice of Termination Request for the Erosion Sedimentation Control General Permit Number ESX11-015-0226, based on the inspection conducted at the site on 2-16-18. The previous inspection conducted at the site by the Department was on 6-5-18. During the 6-5-18 inspection, I had observed trash scattered in the stormwater perimeter ditch at the junction of the pad surface and the base of the site cut slope, and in the stormwater perimeter ditch adjacent to the si

There were 3 properly tagged Marcellus wells staged for production and plumbed to 2 550-gallon plastic tanks that were on a plastic containment. The well cellars were filled with gravel. There were 3 GPUs and 3 produced water tanks onsite and in secondary containment.

The pad was reduced in size and had established vegetation that was greater than 70% cover. No E&S issues were noted.
