2568746 Primary Facility 2017-03-07 Preoperation Inspection No Violations Noted On March 3, 2017, the Department conducted a pre-construction inspection at the SWN Production Co. (SWN) GU-X-Seymour-Pad in Stevens Township, Bradford County and Rush Township, Susquehanna County. Seth Tratthen, Tracy Gregory and Justin Johnson were on site for SWN. Andrew Hobbins, Water Quality Specialist, represented the Department.

The Department walked around the perimeter of the pad and observed the fill and cut slopes to be temporarily stabilized with erosion matting and mulch. Compost filter sock was observed to be installed according to the approved ESCGP-2. Infiltration berms have not yet been installed, but will be in the spring when weather and soil conditions are more favorable. The rock lined ditch that leads to the infiltration berms has been converted to a level spreader for the time being. A compost filter sock will be installed down gradient of the rocked level spreader.

The pad surface has not yet been completed. Aggregate will continue to be placed on the surface. SWN expect

2577703 Primary Facility 2017-03-28 Drilling/Alteration No Violations Noted Arrived at the site at 12:30. Well Service Group Rig # 01 is at the location currently in the process of drilling the Seymour 1H well at a depth of 619 feet. SWN submitted a Drilling Resumption After Break of 30 days or more notification to the Department on 3/24/17. The notice indicates that WSG-1 will be moving onto Seymour 1H GU-X pad. I met with Mike Deming, Senior Regulatory Analyst of SWN, Johnny Hale, site Company Man of SWN and John Hughes, Drilling Superintendent of SWN. They stated the following in terms of site activities. "Drilling of the 1H well commenced on 3-26-17. The surface casing was cemented at 1:45 AM on 3-27-17 at a depth of 581 feet. The intermediate casing will be cemented at 1,640 feet. SDC Rig # 45 will re-enter and drill the well from kickoff point to total depth. WSG 01 will drill the 1H and 3H well to the intermediate string. 2584231 Primary Facility 2017-04-12 Drilling/Alteration No Violations Noted I arrived at the site at 10:25. I met with Mike Deming, Senior Regulatory Analyst of SWN and Dan Hessdorfer, site company man of SWN. I was accompanied by Andrew Hobbins, DEP O&G WQS. SDC Drilling Rig #45 is on the location currently in the process of drilling the Seymour 1H at a depth of 3,672 feet. Mr. Hessdorfer stated the following in reference to site activities. "We are in the process of drilling the vertical section of the well with diesel oil based mud in the Shale Formation above the Tully Formation. The kickoff point is at 5,449. The curve will be landed at 6,768 feet. The well will be drilled to a total of 18,952 feet. Drilling of the 1H well with SDC 45 commenced on 4/10/17 at 12:00 Hours. The 3H well will be re-entered and drilled to total depth upon completion of drilling and cementing of the 1H well." The Department left the site at 12:00.
2607852 Primary Facility 2017-06-14 Routine/Complete Inspection No Violations Noted Arrived at the site at 11:00. The weather conditions were mostly sunny skies with a light breeze with temperatures around 80 degrees. I was accompanied by Andrew Hobbins, PADEP O&G WQS during the inspection. We met with Eddie New, and Corey Pankey, well site supervisors, both consultants of SWN Production CO LLC. According to Mr. New and Mr. Pankey stimulation of the wells was completed as of 6/12/17. The 1H and 3H wells were stimulated. The 1H well had (66) frac stages. SWN is in the process of rigging up for frac plug drill outs and this operation may commence later today or tomorrow. The Department left the site at 12:30.
2619370 Primary Facility 2017-07-19 Routine/Complete Inspection No Violations Noted On July 19, 2017, the Department conducted a routine inspection at the SWN Production Company, GU-X-Seymour-Pad in Stevens Township, Bradford County. Andrew Hobbins, Water Quality Specialist, represented the Department. Brad Wilson with Oil States Services was on site performing flowback operations.

The Department observed 18 tanks on site, only five of which were in service. The secondary containment appeared to be in good condition. All specific conductivity readings taken from the secondary containment and the well pad surface were low. There was no staining observed on the surface of the pad.

Compost filter sock remains at the site and appears to be functioning properly. Vegetation is growing on the slopes of the pad, but is not yet 70% vegetated. Restoration work was continuing on the pipeline within the LOD of the X-Pad.

No violations were observed during the inspection.

2675736 Primary Facility 2017-12-27 Routine/Complete Inspection No Violations Noted On December 27, 2017, the Department conducted a routine inspection at the SWN Production Company LLC GU-X-Seymour-Pad in Stevens Township, Bradford County. Andrew Hobbins represented the Department. Nobody was present for SWN.

The site sign, E & S plan and weekly inspection logs were on site and available for review. The site has perennial vegetation starting to grow, but does not yet meet the 70% requirement. PCSM has been addressed with the recent installation of two infiltration berms. The berms and surrounding disturbed earth were temporarily stabilized with mulch and erosion control matting.

The 1H and 3H wells were in production during the inspection. Both wells were labeled. The 2H and 4H wells had conductor casing set, but were not drilled to total depth. Liner and rig mats were installed around the well heads. One production tank and two GPUs were observed in secondary containment. 2700482 Primary Facility 2018-02-20 Incident- Response to Accident or Event No Violations Noted On February 20, 2018, the Department conducted an incident response inspection at the SWN Production Company LLC, GU-X-Seymour-Pad in Stevens Township, Bradford County. Andrew Hobbins, Water Quality Specialist, and Jon Sassi, Environmental Protection Specialist, represented the Department. Tommy Howard, well site supervisor, and Tracy Gregory, Regulatory Analyst, were on site for SWN. The Department was notified by Steve Moyer on February 18, 2018 that approximately 130 gallons of diesel spilled onto containment. A tear was found in the liner and diesel made its way under the liner and onto the surface of the pad. Mr. Moyer also noted that SWN had contracted Environmental Resource Recovery to remediate the spill.

The Department arrived on site at 1030 hrs. and observed 6 roll-off containers staged on secondary containment. Five containers contained contaminated soil, and one container was filled with liner material. The Department then observed the site of the spill. The excavation area was covere

Water storage tank on location. No spills detected. Site stable. Residual clean-up operation remain for spilled diesel from February 2018 (insp 2696952). No issues detected.
2724923 Primary Facility 2018-05-01 Incident- Response to Accident or Event No Violations Noted Reason for inspection is due to Repsol reporting to the department and SWN, that there was an increase of production water noticed at the Bennett 4H and 5H well's, due to a well communication during the frack process on the Seymour 4H well. During the inspection i spoke with John Sites (EQT Company man). He, as well as the company engineer were unaware of the communication's event that took place on 4/29/2018. The lateral part of this well bore is within 1,000 feet of the Bennett 4H lateral well bore. I ask John if they had noticed a drop in pressures while fracking, he informed me that they did not notice any pressure drops during the frack process on the 4H well.
  • Please note* there were no environmental incident, loss of well control, or other well bore integrity issues related to the increase of water production on the Bennett (Repsol) nor the Seymour (SWN) well pads.

At the time of this inspection SWN was running a wire line on the Seymour 2H well. I was also informed that they have fracked stage 28
