2682601 Primary Facility 2018-01-12 Incident- Response to Accident or Event Violation(s) Noted On 1/5/2018, at approximately 8:45 am a PennEnergy Resources (PennEnergy) contract employee from Rockwater Energy Solutions was operating a telehandler to move a 40-foot long 16" diameter DR11 HDPE plastic pipe adjacent to the well pad's AST containing produced fluids while transporting the 1,165 pound 40-foot long pipe above an obstruction, the pipe slid off the telehandler forks and skidded into the main compartment of the well pad's double walled AST. PennEnergy first became aware of the release on Wednesday morning 1/10/2018 after identifying holes in the liner system subsequent to draining and cleaning.

On 1/10/2018, PennEnergy provided notification to the Department.

0n 1/10/2018, WQS Brian MacQuarrie met onsite with Rick Mays and Joseph Schwab of PennEnergy. Currently a crew is onsite within the tank super-sucking remaining fluids off of upper liner. The liner system consists of four layers of dense plastic liners (two 30 millimeter and two 40 millimeter) as well as three geotextile layers.

2684579 Primary Facility 2018-01-18 Follow-up Inspection Outstanding Violations - Viols Req'd On 1/18/2018, WQS Brian MacQuarrie and WQSS Scott Lux onsite. Meet with Rick Mays of Penn Energy. The purpose of today's inspection was to follow up on impaired water/ brine release from AST which was noted earlier in Inspection # 2682601 on 1/10/2018. The tank has been torn down and staged on pad. The excavation has been completed with approximately 40-45 tons of material removed. Excavation covered with plastic. Materials from excavation staged on plastic adjacent to excavation. Confirmation excavation samples were sent out for analysis.

The project attained a Residential Statewide Health Standard for aluminum, barium, boron, iron, lithium, manganese, selenium, strontium and zinc in the soil.
